Yılda 3 sayı olarak Ocak, Mayıs ve Ekim aylarında yayınlanan Marmara Medical Journal hakemli ve multidisipliner bir dergidir.Gönderilen yazılar Türkçe veya İngilizce olabilir. Değerlendirme süreci Dergiye gönderilen yazılar, ilk olarak dergi standartları açısından incelenir. Derginin istediği forma uymayan yazılar, daha ileri bir incelemeye gerek görülmeksizin yazarlarına iade edilir. Zaman ve emek kaybına yol açılmaması için, yazarlar dergi kurallarını dikkatli incelemeleri önerilir. Dergi kurallarına uygunluğuna karar verilen yazılar Editörler Kurulu tarafından incelenir ve en az biri başka kurumdan olmak üzere iki ya da daha fazla hakeme gönderilir. Editör, Kurulu yazıyı reddetme ya da yazara(lara) ek değişiklikler için gönderme veya yazarları bilgilendirerek kısaltma yapmak hakkına sahiptir. Gönderilen yazıların dergide yayınlanabilmesi için daha önce başka bir bilimsel yayın organında yayınlanmamış olması gerekir. Daha önce sözlü ya da poster olarak sunulmuş çalışmalar, yazının başlık sayfasında tarihi ve yeri ile birlikte belirtilmelidir. Yayınlanması için başvuruda bulunulan makalelerin, adı geçen tüm yazarlar tarafından onaylanmış olması ve çalışmanın başka bir yerde yayınlanmamış olması ya da yayınlanmak üzere değerlendirmede olmaması gerekmektedir. Anatomik terimlerin ve diğer tıp terimlerinin adları Latince olmalıdır. Gönderilen yazılar, yazım kuralları açısından Uluslararası Tıp Editörleri Komitesi tarafından hazırlanan “Biomedikal Dergilere Gönderilen Makalelerde Bulunması Gereken Standartlar “ a ( Uniform Requirements For Manuscripts Submittted to Biomedical Journals ) uygun olarak hazırlanmalıdır. (http://www. ulakbim.gov.tr /cabim/vt) Makale içinde kullanılan kısaltmalar Uluslararası kabul edilen şeklide olmalıdır (http..//www.journals.tubitak.gov.tr/kitap/ma www.marmaramedicaljournal.org knasyaz/) kaynağına başvurulabilir. Birimler, Ağırlıklar ve Ölçüler 11. Genel Konferansı'nda kabul edildiği şekilde Uluslararası Sistem (SI) ile uyumlu olmalıdır. Makaleler Word, WordPerfect, EPS, LaTeX, text, Postscript veya RTF formatında hazırlanmalı, şekil ve fotoğraflar ayrı dosyalar halinde TIFF, GIF, JPG, BMP, Postscript, veya EPS formatında kabul edilmektedir. Yazı kategorileri Yazının gönderildiği metin dosyasının içinde sırasıyla, Türkçe başlık, özet, anahtar sözcükler, İngilizce başlık, özet, İngilizce anahtar sözcükler, makalenin metini, kaynaklar, her sayfaya bir tablo olmak üzere tablolar ve son sayfada şekillerin (varsa) alt yazıları şeklinde olmalıdır. Metin dosyanızın içinde, yazar isimleri ve kurumlara ait bilgi, makalede kullanılan şekil ve resimler olmamalıdır. Özgün Araştırma Makaleleri Türkçe ve İngilizce özetler yazı başlığı ile birlikte verilmelidir. (i)özetler: Amaç (Objectives), Gereç ve Yöntem (Materials and Methods) ya da Hastalar ve Yöntemler (Patients and Methods), Bulgular (Results) ve Sonuç (Conclusion) bölümlerine ayrılmalı ve 200 sözcüğü geçmemelidir. (ii) Anahtar Sözcükler Index Medicus Medical Subject Headings (MeSH) ‘e uygun seçilmelidir. Yazının diğer bölümleri, (iii) Giriş, (iv) Gereç ve Yöntem / Hastalar ve Yöntemler, (v) Bulgular, (vi) Tartışma ve (vii) Kaynaklar'dır. Başlık sayfası dışında yazının hiçbir bölümünün ayrı sayfalarda başlatılması zorunluluğu yoktur. Maddi kaynak , çalışmayı destekleyen burslar, kuruluşlar, fonlar, metnin sonunda teşekkürler kısmında belirtilmelidir. Olgu sunumları İngilizce ve Türkçe özetleri kısa ve tek paragraflık olmalıdır. Olgu sunumu özetleri ağırlıklı olarak mutlaka olgu hakkında bilgileri içermektedir. Anahtar sözcüklerinden sonra giriş, olgu(lar) tartışma ve kaynaklar şeklinde düzenlenmelidir. Derleme yazıları İngilizce ve Türkçe başlık, İngilizce ve Türkçe özet ve İngilizce ve Türkçe anahtar kelimeler yer almalıdır. Kaynak sayısı 50 ile sınırlanması önerilmektedir. Kaynaklar Kaynaklar yazıda kullanılış sırasına göre numaralanmalıdır. Kaynaklarda verilen makale yazarlarının sayısı 6 dan fazla ise ilk 3 yazar belirtilmeli ve İngilizce kaynaklarda ilk 3 yazar isminden sonra “ et al.”, Türkçe kaynaklarda ise ilk 3 yazar isminden sonra “ ve ark. “ ibaresi kullanılmalıdır. Noktalamalara birden çok yazarlı bir çalışmayı tek yazar adıyla kısaltmamaya ve kaynak sayfalarının başlangıç ve bitimlerinin belirtilmesine dikkat edilmelidir. Kaynaklarda verilen dergi isimleri Index Medicus'a (http://www.ncbi.nim.nih.gov/sites/entrez/qu ery.fcgi?db=nlmcatalog) veya Ulakbim/Türk Tıp Dizini’ne uygun olarak kısaltılmalıdır. Tablolar Metin içinde atıfta bulunulan sıraya göre romen rakkamı ile numaralanmalıdır. Her tablo ayrı bir sayfaya ve tablonun üst kısmına kısa ancak anlaşılır bir başlık verilerek hazırlanmalıdır. Başlık ve dipnot açıklayıcı olmalıdır. Sütun başlıkları kısa ve ölçüm değerleri parantez içinde verilmelidir. Bütün kısaltmalar ve semboller dipnotta açıklanmalıdır. Büyütmeler, şekilde uzunluk birimi (bar çubuğu içinde) ile belirtilmelidir. Mikroskopik resimlerde büyütme oranı ve boyama tekniği açıklanmalıdır. Etik Marmara Medical Journal’a yayınlanması amacı ile gönderilen yazılar Helsinki Bildirgesi, İyi Klinik Uygulamalar Kılavuzu,İyi Laboratuar Uygulamaları Kılavuzu esaslarına uymalıdır. Gerek insanlar gerekse hayvanlar açısından etik koşullara uygun olmayan yazılar yayınlanmak üzere kabul edilemez. Marmara Medical Journal, insanlar üzerinde yapılan araştırmaların önceden Araştırma Etik Kurulu tarafından onayının alınması şartını arar. Yazarlardan, yazının detaylarını ve tarihini bildirecek şekilde imzalı bir beyan ile başvurmaları istenir. Çalışmalar deney hayvanı kullanımını içeriyorsa, hayvan bakımı ve kullanımında yapılan işlemler yazı içinde kısaca tanımlanmalıdır. Deney hayvanlarında özel derişimlerde ilaç kullanıldıysa, yazar bu derişimin kullanılma mantığını belirtmelidir. İnsanlar üzerinde yapılan deneysel çalışmaların sonuçlarını bildiren yazılarda, Kurumsal Etik Kurul onayı alındığını ve bu çalışmanın yapıldığı gönüllü ya da hastalara uygulanacak prosedürlerin özelliği tümüyle kendilerine anlatıldıktan sonra, onaylarının alındığını gösterir cümleler yer almalıdır. Yazarlar, bu tür bir çalışma söz konusu olduğunda, uluslararası alanda kabul edilen kılavuzlara ve TC. Sağlık Bakanlığı tarafından getirilen ve 28 Aralık 2008 tarih ve 27089 sayılı Resmi Gazete'de yayınlanan "Klinik araştırmaları Hakkında Yönetmelik" ve daha sonra yayınlanan 11 Mart 2010 tarihli resmi gazete ve 25518 sayılı “Klinik Araştırmalar Hakkında Yönetmelikte Değişiklik Yapıldığına Dair Yönetmelik” hükümlerine uyulduğunu belirtmeli ve kurumdan aldıkları Etik Komitesi onayını göndermelidir. Hayvanlar üzerinde yapılan çalışmalar için de gereken izin alınmalı; yazıda deneklere ağrı, acı ve rahatsızlık verilmemesi için neler yapıldığı açık bir şekilde belirtilmelidir. Hasta kimliğini tanıtacak fotoğraf kullanıldığında, hastanın yazılı onayı gönderilmelidir. Murat Bezer, Nuri Aydın, Osman Güven Ortopedi ve Traumatoloji Anabilim Dalı, Tıp Fakültesi, Marmara Üniversitesi, İstanbul, Türkiye ÖZET Amaç:Rotator kılıf yırtığı bulunan hastalarda sağlam ve yırtık kılıf bulunan omuzda humerus başının glenoide göre superior’a kaymasının tamir sonrası prognostik kriter olarak kullanılabilirliği araştırıldı. Hastalar ve Yöntem:Tek taraflı tam kat rotator kılıf yırtığı tanısı konulan 27 hasta (14K, 13E) çalışmaya dahil edildi. Ortalama yaş 56 (Aralık: 32-71) idi. Ameliyat öncesi her iki omuz glenohumeral eklemin AP grafileri çekildi.Humerus başının yukarı kayma miktarı (∆e) ölçüldü. Kayma miktarı 0-3 mm olan hastalar Grup A, 4-5 mm olan hastalar Grup B olarak sınıflandı. Açık rotator kılıf tamiri yapıldı.Değerlendirmede Constant omuz skorlaması kullanıldı. Bulgular: Supraspinatus, infraspinatus ve subskapularis tendon yırtıkları sırasıyla 27 hastada (%100), 11 hastada (%40.7) ve 6 hastada (%22.2) saptandı.19 hasta A grubunda, 8 hasta B grubundaydı. İzole supraspinatus yırtığı bulunan hastaların hepsi (n=10) ve supraspinatus, infraspinatus yırtığı bulunan 11 hastanın 9'u A grubundaydı. Supraspinatus, infraspinatus ve subskapularis yırtığı bulunan hastaların hepsi (n=6) B grubundaydı. Ortalama preoperatif Constant skoru Grup A'da 48.6, Grup B'de 21.5 olarak tespit edildi. Aradaki fark istatistiksel olarak anlamlı bulundu (p<0.001). Ortalama postoperatif Constant skoru Grup A'da 75.3 grup B'de 69 idi. Her iki grup karşılaştırıldığında aradaki fark istatistiksel olarak anlamlı bulundu.(p<0.05) Ameliyat öncesi ve sonrası Constant skorlarıyla ∆e değeri arasında ters bir ilişki olduğu tespit edildi. Sonuç:Rotator kılıf yırtığından şüphelenilen hastalarda direk radyografide humerus başının yukarı kayması tamir sonrası prognozu göstermesi açısından önemlidir. ABSTRACT Objective:Superior excursion of the humeral head was investigated to determine whether a relationship exits between the degree of humeral head excursion and the prognosis after surgical repair. Patients and Methods:Twenty-seven patients with one sided full thickness rotator cuff tears were included in this study.The mean age was 56 (Range:32-71) Preoperative AP x rays of both shoulders were taken. Superior excursion of the humeral head was measured.(∆e). Group A includes 0-3 mm excursion, Group B 4-5 mm. All patients underwent open cuff surgery. Constant scores were used for evaluation. Results: Supraspinatus, infraspinatus and subscapularis tears were found in 27 patients (%100), 11 patients (40.7%), and 6 patients (22.2%) respectively.19 patients were in Group A, 8 patients were in Group B. Of the patients with supraspinatus tears(n=10), 9 of 11 patients with both supraspinatus and infraspinatus tears were in Group A.The patients with supraspinatus, infraspinatus and subscapularis tears (n=6) were all in Group B.The mean preoperative Constant Score was 48.6 in Group A, 21.5 in Group B.The mean postoperative Constant Score was 75.3 in Group A, 69 in Group B.There was a significant difference between the two groups statistically (p<0.05). A reverse relation was found to be between pre/postoperative Constant Scores and ∆e. Conclusion:Superior excursion of the humeral head is an important criteria for the postoperative prognosis. GİRİŞ Omuz ekleminin tam fonksiyonu için sağlam bir rotator kılıf gereklidir. Rotator kılıfın omuz üzerinde üç önemli etkisi vardır. Bunlar humerus başını glenoid’e bastırmak, eklem kontakt basıncını artırmak ve humerus başını glenoid merkezinde odaklamaktır.1 Deltoid kası, supraspinatus ve infraspinatusla beraber çalışarak omuza abdüksiyon hareketini yaptırır.2-5 Abdüksiyonun başlangıcında deltoid kası vektörel kuvveti humerus başını yukarı doğru çeker. Rotator kılıf ise ters güç uygulayarak başın glenoid’den yukarı çıkmasını engeller.5,6 Rotator kılıf yırtığı veya zayıflığı durumunda deltoid gücünün karşılıksız kalması nedeniyle humerus başı abdüksiyon esnasında yukarı doğru kayar7. Ameliyat öncesinde her iki omuz glenohumeral eklemin anteroposterior (AP) radyografileri çekildi. Standart çekim için, radyografiler hasta dik pozisyonda ve 30 derece aktif abdüksiyon esnasında skapular planda çekildi. Işın glenoid planına paralel skapula planına dik olacak şekilde ayarlandı. AP görüntüde glenoide paralel pozisyonda superior ve inferior kenara teğet olacak şekilde referans Y ekseni çizildi. Humerus başının geometrik merkezinden (Oh) ve glenoid merkezinden (Os) geçen Y eksenine dik çizgiler çizildi. Oh ve Os arasındaki mesafe ölçüldü 5 (Şekil 1). Glenoid yüzeyinin merkez noktası referans alınarak humerus başının bu noktanın altında olduğu durum negatif kayma, üzerinde olduğu durum pozitif kayma, değeri olarak kaydedildi. Oh ve Os arasındaki mesafe sağlam ve rotator kılıf yırtığı bulunan omuzda ölçülerek ‘e' değeri olarak belirtildi. Sağlam omuzla rotator kılıf yırtığı bulunan omuzdaki ‘e' değer farkı (∆e) kayma miktarı olarak değerlendirildi. Hastalar ∆e değerlerine göre 2 gruba ayrıldılar. Grup A'yı kayma miktarı 0-3 mm olan hastalar, Grup B'yi 45 mm olan hastalar oluşturdu. Postoperatif Constant skorları ortalama 46 aylık takip sonucunda belirlendi ve humerus başının kayma miktarıyla ameliyat öncesi ve sonrası Constant skorları arasındaki ilişki istatistiksel olarak değerlendirildi. Rotator kılıf yırtığı bulunan hastalarda direk radyografide akromiohumeral mesafedeki (AHM) daralma ilk olarak Golding8 tarafından tanımlanmıştır. Golding normal omuzlarda AHM'in 7-13 mm olduğunu tespit etmiştir. Daha sonra sağlam rotator kılıfı bulunan hastalarda Cotton ve Rideout4 6-14 mm, Weiner ve Macnab9 7-14 mm'yi normal kabul ederken yırtık olan rotator kılıfta bu mesafenin 5 mm ve altı olduğunu belirtmişlerdir. Son olarak Deutsch ve ark'nın5 çalışmasında rotator kılıf fonksiyonunun humerus başı pozisyonuna etkisi araştırılmış ve rotator kılıf yırtıklarında AHM yerine humerus başının glenoid'e göre yukarı hareketi incelenmiştir. Bahsedilen çalışmaların hiçbirisinde AHM veya humerus başının yukarı doğru kaymasıyla prognoz arasındaki ilişki incelenmemiştir. Bu çalışmanın amacı, direk grafiden ölçülen humerus başının superior’a kayma miktarının rotator kılıf ameliyatı öncesi ve sonrasında Constant skoruyla bağlantısının araştırılması ve bu ölçümün prognostik faktör olarak kullanılıp kullanılamayacağının incelenmesidir. HASTALAR VE YÖNTEM 1990 ve 2000 yılları arasında manyetik rezonans görüntüleme veya artrogramla tanı konulan tek taraflı tam kat rotator kılıf yırtığı ile başvuran 27 hasta (14 kadın, 13 erkek) çalışmaya dahil edildi. Diğer omuzu ile ilgili rotator kılıf problemi hikayesi, semptomu veya muayene bulgusu tespit edilen hastalar çalışmaya dahil edilmedi. Hastaların Constant skorları ameliyat öncesinde tespit edildi. Etkilenen tendonlar ve büyüklükleri cerrahi esnasında kaydedildi. Tüm ameliyatlar aynı cerrah tarafından yapıldı. İstatistiksel karşılaştırmada ANOVA, TukeyKramer çoklu karşılaştırma ve unpaired t-testleri kullanıldı. TARTIŞMA Omuz abdüksiyonu deltoid ve rotator kılıfın ortak çalışmasıyla sağlanır.1,5,6,10,11 Deltoid’in kasılmasıyla humerus başı glenoid’den çıkmaya yönelirken buna karşılık rotator kılıf başı aşağı doğru bastırarak glenoid yuvada kalmasını sağlar. Bu denge abdüksiyon için yeterli gücü humerus’a iletir2,12,13. Rotator kılıfta büyük bir yırtığın olması durumunda deltoid’in yukarı çekme gücü karşılıksız kalacak ve bu denge bozularak humerus başının superior’a kayma meydana gelecektir5. Rotator manşet tamirinin, omzun pasif ve aktif yardımlı egzersizlerine izin verecek ölçüde sağlam olması gerekir14. Direk radyografide rotator kılıf yırtığı tanısında akromiohumeral mesafenin önemi bir çok araştırmacı tarafından tartışılmıştır4,8,9,15. İlk olarak Golding 150 hastada sağlam tarafla karşılaştırmalı ölçüm sonuçlarını yayınlamıştır. Bu çalışmada AHM sağlam omuzda 7-13 mm arası bulunmuştur8. Cotton ve Rideout omuz ekleminde radyolojik ve patolojik çalışmalar yaparak bu değişikliklerin rotator kılıf yırtığı ile ilişkisini değerlendirmişlerdir4. AHM'nin sağlam rotator kılıfta 6-14 mm olduğu, tam kat rotator kılıf yırtığında ise 1-4 mm olduğu gösterilmiştir. 1970 yılında Weiner ve Macnab, normal AHM nin 7-14 mm olduğunu ve 5 mm altında rotator kılıf yırtığının düşünülmesi gerektiğini 9 belirtmişlerdir . BULGULAR Hastaların ortalama yaşı 56 (Aralık: 32-71) idi. Postoperatif Constant skorları ortalama 46 aylık takip sonucunda belirlendi. İntraoperatif olarak supraspinatus, infraspinatus ve subskapularis tendon yırtıkları sırasıyla 27 hastada (%100), 11 hastada (%40.7) ve 6 hastada (%22.2 ) vardı. Infraspinatus ve subskapularis yırtıkları eşlik eden yırtıklar olarak tespit edildi. Teres minör yırtığına rastlanmadı. Rotator kılıf yırtığı bulunan omuzlarda humerus başının glenoid'e göre pozisyonu ortalama (+) 0.03 mm (Aralık:0.020.04) iken sağlam omuzda bu değer ortalama (-) 2 mm (Aralık: 1 – 3 mm) olarak bulundu. Aradaki fark istatistiksel olarak anlamlıydı (p<0.0001). Preoperatif ∆e değeri 19 hastada 0-3 mm (Grup A), 8 hastada 4-5mm (Grup B) bulundu. İzole supraspinatus yırtığı bulunan hastaların hepsi (n=10) ve supraspinaus, infraspinatus yırtığı bulunan 11 hastanın 9'u A grubundaydı. Diğer yandan supraspinatus, infraspinatus ve subscapularis yırtığı bulunan 6 hastanın hepsi B grubundaydı. Ortalama preoperatif Constant skoru Grup A'da 48.6, Grup B'de 21.5 olarak tespit edildi. Aradaki fark istatistiksel olarak anlamlı bulundu (Şekil 2A-B). Ameliyat öncesi ve sonrası Constant skorlarıyla ∆e değeri arasında ters bir ilişki olduğu tespit edildi. Yukarı kaymanın (∆e) fazla olduğu hastaların (Grup B) geç dönem fonksiyonel sonuçları, yukarı kaymanın daha az olduğu hastalara göre daha kötü olarak saptandı (p<0.05). Hamada ve ark. masif supraspinatus ve infraspinatus yırtığı artrografi ile doğrulanan hastalarda ortalama AHM'yi 4.4 mm (aralık: 0-9 mm) bulmuşlardır15. Bu çalışmamızda, humerus başının yukarı kaymasıyla ameliyat öncesi Constant skoru arasında ters ilişki bulunmuştur. Rotator kılıf tamiri sonrasındaki takip döneminde Constant skorları tekrar gözden geçirilmiştir. Bunun sonucunda, ameliyat öncesi ölçümlerde humerus başında 3 mm'den az yukarı kayma bulunan hastaların ortalama 46 aylık kontrollerinde Constant skorlarında diğer grupla karşılaştırıldığında anlamlı fark bulunmuştur. İki gruba da aynı kişi tarafından ve standart kılıf tamiri yapılmış olmasına rağmen preoperatif dönemde humerus başının yukarı kaymasının daha fazla olduğu grubun (Grup B) Constant skorunun takip döneminde de düşük olduğu gözlenmiştir. Bu nedenle, ameliyat öncesi sağlam omuzla karşılaştırılarak yapılacak olan humerus başının glenoid'e göre yukarı hareket ölçümü rotator kılıf ameliyatı sonrasında prognozun belirlenmesi açısından önem göstermektedir. Deutsch ve ark subakromial sıkışma sendromu ve rotator kılıf yırtığı bulunan hastalarda humerus başının yukarı kayma miktarını radyolojik olarak değerlendirmişlerdir5. AHM'yi ölçmek yerine Poppen ve Walker16 farklı omuz açılarında glenoid merkezine göre humerus başının pozisyonu ölçülmüştür. Normal omuzda abdüksiyon başlangıcında humerus başı pozisyonunda belirgin bir değişiklik görülmez. Bununla birlikte kılıf yırtığı ve subakromial sıkışması olan omuzlar normal omuzlarla kıyaslandığında omuz abdüksiyonunun ilk 40 derecesinde humerus başının glenoid'e göre yukarı doğru kaydığı görülmüştür. Bu nedenle çalışmamızda rotator kılıf kaslarının dinamik olarak abdüksiyona katkı sağladığı 30 derece aktif abdüksiyondaki radyografilerden yapılan ölçümler kullanılmıştır. Abdüksiyon esnasında humerus başının hareketine paralel olarak glenoid yüzey de mediale ve yukarı doğru hareket eder16. Bu nedenle, humerus başının glenoid üzerindeki yukarı hareketi glenohumeral uyumu yansıtması açısından akromiohumeral mesafeye göre daha uygundur. Çalışmamızda AHM'yi referans alan çalışmalardan farklı olarak humerus başının glenoid yüzeye olan konumu baz alınmıştır 4,8,9,15. Bu çalışmamızda yapılan kılıf tamirlerinin tümünün sağlam kaldığı kesin olarak bilinmemesine rağmen (MRI veya diğer yöntemlerle kontrol yapılmadı) literatürde rerüptür oranlarının düşük olması ve takiplerde rekürrens bulgularının olmaması rerüptür açısından bir fikir verebilir. Hastaların asemptomatik olması da bunu desteklemektedir. Bu çalışmada humerus başının yukarı kayması ile preoperatif ve postoperatif Constant skorları arasındaki ilişki değerlendirilmiştir. Ameliyat öncesinde sağlam omuzla karşılaştırıldığında humerus başında glenoid'e göre 0-3 mm yukarı kayma tespit edilen hasta grubunun (Grup A) 4-5 mm kayma bulunan gruba göre (Grup B) Constant skorlarının istatistiksel olarak daha iyi olduğu bulunmuştur. Bu farklılığın nedeni kaymanın az olduğu Grup A'daki rotator kılıf yırtıklarında etkilenen tendon sayısındaki değişiklikler ya da rotator kılıfı oluşturan kasların fizyolojik özellikleriyle ilişkili olabilir. Subskapularis rotator kılıfın en kuvvetli kasıdır17. Abdüksiyon sırasında humerus başını inferior ve posteriora çekerek stabilize eder11,18,19 .Bu güçlü kasın yırtılması, humerus başının diğerleriyle karşılaştırıldığında daha fazla yukarı kaymaya neden olmuş olabilir. Bunun yanında infraspinatus relatif olarak daha zayıf bir kastır. Subskapularis rotator kılıf momentinin % 53'üne katkı sağlarken infraspinatus momentin % 22'sine katkıda bulunur10 .Sonuç olarak bu kasın yırtılması humerus başının daha az miktarda yukarı kaymasına neden olacaktır. Sonuç olarak tek taraflı tam kat rotator kılıf yırtığından şüphelenilen hastalarda direk radyografide humerus başının yukarı kayması tamir sonrasındaki prognozu göstermesi açısından önemlidir. Sağlam omuzla karşılaştırıldığında humerus başının glenoid'e göre 3'mm den fazla yukarı kaydığı omuzlar preoperatif ve postoperatif Constant skorlarıyla ters ilişkilidir ve postoperatif kötü sonuç için bir gösterge olabilir. Methods: The sample consisted of 17 (11 males, 6 females), right-handed, healthy, volunteer participants. Event-related activity was recorded under the omitted stimulus paradigm. Alpha response was obtained through digital filtering. The alpha response of each participant was separately analyzed. Results: The ARIMA(0,2,1) model was found to be applicable to alpha response that was evoked by stimulus omission. ARIMA (0,2,1) shows that this activity is short-lasting and it fluctuates stochastically. Conclusion: The present study showed that the alpha responses of single participants obtained under the omitted stimulus paradigm were best modeled by the ARIMA (0, 2, 1) type of time series model. Yöntem: Çalışan örneklem 17 (11 erkek, 6 kadın) sağ elini kullanan, sağlıklı gönüllülerden oluşmuştur. Olay ilişkili aktivite, atlanmış uyarıcı paradigması altında kaydedilmiştir. Alfa tepkisi sayısal filtreyle elde edilmiştir. Her katılımcının alfa tepksi ayrı ayrı analiz edilmiştir. Bulgular: Yapılan çalışma göstermektedir ki her bir katılımcının atlanmış uyarıcı paradigması için alfa tepkisi, ARIMA (0,2,1) tipindeki zaman serisiyle en iyi modellenebilmektedir. Sonuç: Yapılan çalışma atlanmış uyarıcı paradigması altında elde edilmiş, bir katılımcının alfa tepkilerinin en iyi ARIMA (0, 2, 1) zaman serisiyle modellendiğini göstermiştir. The cutoff frequencies of these filters are obtained in a response-adaptive way since they are determined from the limits of the resonant selectivities in the amplitude characteristics that FFT provides1,19,20. Digital filtering uses a function for extracting the sinusoidal components from the analog timedomain waveform. In signal processing, also, filters are used for extracting some parts of the signal, such as those components that occur within a certain frequency range. They are also used for removing unwanted parts of the signal, such as random noise. Event-related potentials (ERPs) are time-varying waveforms and they represent the neuroelectric activity of the brain in response to sensory stimulation or to execution of a motor, cognitive, or psychophysiological task. ERPs are timelocked to events. This event may be a sensory stimulus (such as a visual flash or an auditory sound), a mental event (such as recognition of a specified target stimulus), or the omission of a stimulus (expectancy of a stimulus which however is omitted). The spontaneous activity of the brain, the electroencephalogram (EEG), and the ERPs are continuous over time. Accordingly, they are analog signals. Recording the analog electrical activity into a computerized environment is called digital transformation or digitalization. In digital transformation, electrical activity is recorded in equally spaced time intervals. Statistical techniques can also be used for studying any time varying activity. According to statistics, a time series is composed of successively appearing data points which are typically spaced apart in uniform time intervals. The main goal of time series analysis is to identify and understand the nature of time-dependent events. There are several statistical techniques whereby a time series can be modeled. One of the most studied statistical time series models is that of Box and Jenkins21. This Autoregressive Integrated Moving Average (ARIMA) model employs three sequential processes: the autoregressive (AR) process, integrated (I) process, and moving average (MA) process. The technique is hence called the ARIMA model, a synthetic term that combines the names of the specified subprocesses. The Box and Jenkins model requires stationary time series data for identifying and analysing, which means no change at mean and variance of series according to time. Any analogue or digital time domain activity can be represented by sinusoids. Frequency analysis involves the conversion of the data in the timedomain to frequency domain. A widely used technique of frequency analysis, is the fast Fourier Transform (FFT) (Jean Baptiste Joseph Fourier, 1807). The purpose of FFT is to decompose a complex time series with cyclical components into a few underlying sinusoidal (sine and cosine) functions of particular wavelengths (oscillations). The transformation of the data from the timedomain to the frequency-domain finds the frequency components whose superposition produce the time-domain waveform1. FFT thus demonstrates that the time-domain waveform is represented with frequency components, which are called event-related oscillations (EROs). In this study one of the brains oscillations called alpha activity is formulated with statistical time series. Autoregressive process (AR) is used on a time series where observations are serially dependent. In such data, one can estimate a coefficient or a set of coefficients that describe consecutive elements of a series from the specific, time-lagged previous elements. Integration is a parameter, which is computed for the series after it is differenced once. Each integration order corresponds to differencing the series being forecast. A first-order integrated component means that the forecasting model is designed for the first difference of the original series. A second- order component corresponds to using second differences. An integration parameter is used for obtaining stationary time series. An integration parameter that equals 1 denotes that the time series is increasing linearly, one that The advantage of FFT is the simultaneous demonstration of the frequency components that make up the time-domain waveform2-8. A new and/or pioneering method of analysis involves the simultaneous description of the frequency components in the time-frequency domain. Among these are the various forms of wavelet transform, wavelet entropy, Wigner transform, Cohen’s class of distributions, shorttime Fourier transform and time-frequency component analyzer9-18. The moving average process (MA) is independent from the autoregressive process. Each observation in a time series can also be affected by an error or errors in the past. Each observation can thus be estimated from the previous errors. down, 12 dB /octave). It was recorded with a sampling rate of 512 Hz and a total recording time of 2000 ms. The pre- and post-stimulus interval were each 1000 msec. The single sweeps of each participant (range: 12 - 29 sweeps for each participant) were digitally band-pass filtered in the alpha band range (8-12 Hz)22. The brain electricity is a time–varying activity. The aim of this study was to test whether the alpha oscillation (8-12 Hz) which is obtained through the digital filtering of the ERPs that are recorded under the omitted stimulus paradigm can be formulated through ARIMA models. RESULTS METHODS Calculations for digital filtering were performed under Octave for Linux (GNU license), and statistical analysis was made under R for Linux (GNU license). Graphics were prepared using SPSS 11.5 (licensed for Marmara University, Department of Biostatistics). Participants Model Fitting Procedures The sample consisted of 17 (11 males, 6 females), right-handed, voluntary participants. Mean age of participants was 22.5 and the standard deviation was 2.3 (min.: 20, max.: 27). The participants were reportedly healthy. Hearing level, which was investigated using computerized audiometric testing, was found to be within normal limits. One of the participants was called ALGU in short (nickname) and ALGU’s data was used as a sample. The time series analysis method of Box and Jenkins involves special procedures. First ofall, the time series must be converged into a stationary form according to time. Afterwards, the converted time series are modeled with the help of autocorrelation function and partial autocorrelation function. Autocorrelation function (ACF) and partial autocorrelation function (PACF) are the major tools in the Box and Jenkins method. ARIMA process is thus determined by ACF and PACF graphics. Autocorrelation is a kind of correlation coefficient. However, instead of a correlation between two different variables, the correlation is between two values of the same variable at consecutive time lags (e.g. Xt and Xt+k etc.). The partial autocorrelation removes the effect of shorter autocorrelation lags from the correlation estimate at longer lags. The partial autocorrelation at lag k is the autocorrelation between Xt and Xt-k that is not accounted for by lags 1 through k-1. Electrophysiological Procedures Electrophysiological responses were obtained under the repetitive, omitted stimulus paradigm22. Auditory stimuli were presented over the earphones. The frequency of the auditory stimuli was 1000 Hz. The amplitude and duration of the auditory stimuli were 65 dB and 50 msec. Stimuli were presented at the rate of 2/sec. Stimulation consisted of blocks of five stimuli; in each block, four consecutive stimuli were presented and the fifth was omitted. The task of the participant was to decide on the time of occurrence of the omitted stimulus. The first stage of the analysis involved the calculation of unfiltered, single sweep ERPs and the calculation of filtered ERPs (f-ERP) that show the alpha response. In the next stage, the autocorrelations and partial correlations were calculated for single sweeps. The autocorrelation and partial correlation graphics are shown in Figure 1. Electroencephalographic (EEG) activity was recorded over the Fz recording site (ref: linked earlobes; ground: forehead) of the 10-20 system under eyes-open condition. Bipolar recordings were made of electrooculogram and electromyogram for monitoring eye movement artifacts and muscle artifacts, respectively. Epochs with artifacts were discarded through online rejection (of responses whose amplitudes exceed ±50 mV) and offline rejection (through visual inspection). The EEG was amplified and filtered with a bandpass between 0.16 - 70 Hz (3 dB The ACF and PACF of each participant showed that the function represented by the sweeps did not have a stationary time series trend. Thus, in the third stage, the time series model that best fits these sweeps was investigated. Linear exponential smoothing models (stochastic fluctuations) are ARIMA models, which use two nonseasonal differences in conjunction with MA terms. The second difference of a series Zt is not simply the difference between Zt and itself lagged by two periods, but rather it is the first difference of the first difference. A second difference of a discrete function is analogous to a second derivative of a continuous function; it measures the "acceleration" or "curvature" in the function at a given point in time. The ARIMA (0,2,1) model calculated for the participant, “ALGU”, could be expressed in the form of the following mathematical equation: Zt= 2Zt-1- Zt-2-0.6012at-1+ at where Zt is last observed time series value, Zt-1 is only one lag before observed time series value, Zt2 is two lag before observed time series value, at-1 is a residual term of Zt-2 and Zt-1, and at is a residual term of Zt-1 and Zt. Moreover the ARIMA (0,2,1) coefficient (0.6012) is significant at the p<0.01 level. The calculated coefficients for the data of the present study were, accordingly, optimally obtainable values (Table I). exponential smoothing models were tested. None of these models sufficiently explained the ACF and PACF. Thus, in the fourth stage, first-order difference was calculated whereafter the ACF and PACF were recalculated. Figure 2 shows that, in spite of the first-order difference manipulations, a stationary time series trend could still not be achieved. At the fifth stage, a second-order difference was calculated whereafter the ACF and PACF were again recalculated. Figure 3 shows that, by taking the second-order difference, the time series gained a stationary character. This manipulation involves the usage of the Autoregressive Integrated Moving Average Model (ARIMA). The parameters of the ARIMA were (0,2,1). These values indicate, respectively, that the data did not show an autoregressive structure (AR: 0); that is, it fluctuated (I: 2); and the data were described by only one moving average coefficient for the residual (MA: 1) that varied between 0.60 and 0.88. Statistically, the coefficients of the moving average for the residual should be between 0.5 and 0.9. According to Box and Jenkins, ARIMA(0,2,1) fits short-lasting data of sudden appearance21. The event-related alpha oscillation in fact terminates right after the decision of the participant with respect to the temporal point at which the omission occurred. Statistical modeling techniques such as those used in analyzing time series require model checking. Model checking is a test of the model adequacy. In the time series methodology of Box and Jenkins, model adequacy is checked by analyzing the residuals of time series. Residuals represent the difference between the values that areestimated from the model and those observed in the time series. The residuals have to be in the form of white noise. Furthermore, the values for the autocorrelation function and the partial autocorrelation function have to be nonsignificant at all lags. This would show that the residual (white noise) is a stationary time series or a stationary random process with zero autocorrelation and zero partial autocorrelation. This procedure is realized using Portmanteau test statistics. A portmanteau (most of the time called Q-Statistics) test is a test for serial correlation up to the specified lag (order). In the present study, the graphic representation of the systematic residual in Fig. 4 was calculated mathematically with also the Portmanteau Statistics21. • The coefficient for I was “2”. This shows that the data has a fluctuating character. The eventrelated alpha response is a fluctuating brain signal. • The coefficient for MA was “1”. This indicates the existence of a stochastic process that can be described by a weighted sum of white noise error and the white noise error in the preceding periods. In the digitally filtered alpha response, each data point on the alpha waveform is determined with the previous ones. Furthermore, brain neuroelectricity is a complex phenomenon and no recording is devoid of methodological and technical contamination, and the contamination of brain activity that unrelated to that under experimental investigation. The null hypothesis of Portmanteau test statistics is that all autocorrelation values are equal to zero. A p-value smaller than 0.05 justifies the rejection of the null hypothesis and shows that the time series is not in the form of white noise. According to Table I, the p-values are larger than 0.05, this shows that the difference between observed values and those estimated from the ARIMA model are nonsignificant. Accordingly, the residuals in the present data are in the form of white noise. The foregoing coefficients of the ARIMA model fits in with the electrophysiological representation of expectancy in humans. The process of expecting an event terminates with its appearance (thus, AR=0) and the alpha response, the best electrophysiological representation of expectancy, characteristically oscillates within the 8-12 Hz frequency range (thus, I=2). Finally, mankind is not a perfect machine; its responses are not strictly precise and not 100 % reliable; thus “error” of recording is to be expected in human responses (thus MA=1). These points show the statistical ARIMA model can represent human electrophysiological responses in cognitive processing. DISCUSSION ARIMA (0,2,1) models describe a short-term process that ends rapidly. It is not possible to estimate the future processes with an ARIMA (0,2,1) model; such a model adequately describes only the specific time interval for which the model is designed. The present study showed that, among the time series models, ARIMA (0, 2, 1) best fitted the omitted stimulus alpha response of single participants. As is pointed out below, this finding is in line with the phenomenology of the omitted stimulus response22. REFERENCES 1. 2. Çalışma grubunu 9-15 aylık bebeği olan ve farklı eğitim seviyesi, sosyal ve ekonomik yapıdan 40 anne oluşturmuştur. Yöntem:Çalışmanın verisi, yarı yapılandırılmış sorulardan oluşan bir akış formunun kullanıldığı derinlemesine görüşmeler ile toplanmıştır. Bulgular:Analiz sonrasında, görüşmeler sırasında annelerin ifade ettikleri şu ana temalar altında toplanmıştır:1Annelerin anne sütü verme kararını etkileyen süreçler, 2-Anne sütü ile ilgili bilgi ve inanışlar, 3-Emzirme uygulamaları, 4-Annelerin ek besin başlama kararını etkileyen durumlar, 5-Annelerin ek besinlerle ilgili bilgi ve tutumları. Yurdumuzda emzirme alışkanlığı yaygındır. Çalışmamızda da annelerin tümü bebeklerini emzirmişlerdir. Ancak anne sütünün yeterliliği konusundaki endişeleri ve sosyal baskılar ek gıdalara erken başlanmasına neden olmaktadır. Eğitimler, anne sütü yeterliliğinin değerlendirilmesinde bebeğin kilo alımının esas belirteç olduğu ve anne sütünün tek başına 6 ay yeterli olduğu yönünde vurgu yapmalıdır. Ek gıdaların seçiminde ailelerin alım gücü etkili olmakla birlikte, kısıtlı olanakların uygun şekilde kullanılması için ek gıdaların niteliği ve başlama zamanları konusunda yeterli bilgisi olmadığı görülen annelere danışmanlık verilmesi yararlı olacaktır. Anneler bebek beslenmesi konusunda sağlık çalışanları tarafından birebir görüşmeler ve basılı materyallerle eğitilmeyi tercih etmektedir. Sonuç:Bebek beslenmesinin iyileştirilmesi için topluma yönelik programların hazırlanmasından önce annelerin düşünce, inanış ve gereksinimlerinin belirlenmesi yararlı olacaktır. Forty mothers with infants aged 9-15 months- and with varing different backgrounds in education, social and economic structure, formed the study groups. Method:The data was collected by means of depth interviews useing of a semi-structured questionnaire. Results:After the analysis the mothers’ words during the interviews were classified under these main themes: 1Procedures affecting mothers’ breastfeeding decision 2-Knowledge and beliefs regarding breast-milk, 3Breastfeeding practices, 4-Factors influencing the decisions of feeding with non-breast milk products 5-Mothers’ knowledge and attitudes to complementary nutritients/foods. Breastfeeding is a common habit in Turkey. In our study all of the infants were breastfed. But the mother’sanxiety about the sufficiency of breast-milk and social pressure leads to starting with complementary nutritients/foods early. Education must emphasize exclusive breastfeeding in the first 6 months and following the weight gain of the infant as the major indicator for assessment of its sufficiency. Although the economic level of the family affects the selection of the complementary foods, supervision is necessary for mothers who do not know enough about the quality of the foods and time of addition to the diet. Mothers prefer one to one interviews with health professionals and using printed materials about infant nutrition. Conclusion:Before preparing community-based programs to improve infant nutrition, the mothers’ opinions, beliefs and needs, should be taken into account. Örneklem grubu içinde ev hanımı ve eğitimi ilkokul düzeyinde olan annelerin yanı sıra, orta, lise ve yüksek öğrenim görmüş ve çalışan anneler ile ekonomik düzeyi kötü olan annelerin de yer almasına özen gösterilmiştir. Obezite de bir beslenme sorunu olmakla birlikte bu çalışmaya dahil edilmemiş, ülkemizde süt çocuklarında daha büyük sorun olan erken, yanlış ve yetersiz ek gıdalarla beslenme ve anne sütünden yeterince yararlanamama konusunun araştırılması hedeflenmiştir. Bu amaçla Türkiye’de bebek beslenmesi ile ilgili sorunlar konusundaki yayınlar taranarak ilgili konuda açık uçlu sorular içeren bir görüşme formu hazırlanmıştır. Annelerin, anne sütü, ek gıdalara geçiş, ek gıdaların niteliği konusundaki bilgileri, uygulamaları ve bu bilgileri nerelerden edindikleri sorulmuştur. GİRİŞ Beslenme durumunun değerlendirilmesinde Dünya Sağlık Örgütü’nün de önerdiği üç gösterge kullanılmaktadır1. Yaşa-göre-boyun düşük olması beslenme sorununun kronik olduğunu, boya-göreağırlığın düşük olması akut beslenme yetersizliğini gösterir; yaşa-göre ağırlığın düşük olması hem akut hem de kronik beslenme yetersizliğinde görülebilir1. Yaşa-göre-boy endeksine göre, referans grubun ortanca değerinden eksi iki standart (-2SD) sapma gösteren çocuklar, yaşlarına göre kısa (bodur“stunded”) kabul edilmektedir. Türkiye Nüfus ve Sağlık Araştırması (TNSA) 2003 verilerine göre 24-59 aylık çocukların yaklaşık yüzde 15’i bodurdur2. TNSA 1998 verilerine göre çocukların %2,3’ünün boylarına göre, %9,7’sinin de yaşlarına göre düşük ağırlıklı olduğu, genel malnütrisyon sıklığının %8.7 olduğu saptanmıştır3. Tüm bu göstergeler, ülkemizdeki bebek beslenmesi uygulamalarında önemli sorunlar olduğunu yansıtmaktadır. Ülkemizde süt çocukluğu ve erken çocukluk dönemindeki mortalitenin yüksek oluşundan başlıca malnütrisyon-enfeksiyon kısır döngüsü sorumludur4. 1989’da Hayran ve arkadaşlarının Kocaeli il merkezinde 0-6 yaş grubu çocuklarda yaptığı araştırmada, çocukların %32’sinin yaşa göre ağırlıklarının normalin altında olduğu, bu durumun anne eğitim düzeyinin düşüklüğü ve çocuk sayısının artışı ile paralellik gösterdiği bulunmuştur5. Bizim çalışmamız da ülkenin değişik yerlerinden göç alma ve sanayi merkezi olma özelliği nedeniyle farklı sosyo-kültürel yapıları içeren Kocaeli ilinde yapılmış ve annelerin süt çocuğu beslenmesiyle ilgili bilgi, inanış ve uygulamaları, derinlemesine bireysel görüşme yönteminin kullanıldığı niteliksel bir araştırmayla değerlendirilmiştir. Görüşmelerin tamamı niteliksel araştırma konusunda eğitim almış olan bir kadın araştırmacı tarafından yapılmıştır. Çalışma konusunda bilgilendirilen annelerden gönüllü olanlarla görüşülmüştür. Görüşmeler hastane veya sağlık ocağının bir odasında gerçekleştirilmiş, anne, görüşmeci ve bazen de çocuk dışında kimse odaya alınmamıştır. Görüşmeler annelerin izniyle teybe kaydedilmiştir. Görüşme yaklaşık 60 dakika sürmüş, aynı gün teyp kayıtları yazıya dökülmüştür. Yöntemde önerildiği şekilde, görüşmeci az konuşup daha çok dinlemiş ve anneyi yönlendirmemeye özen göstermiştir. Başlangıçta belli bir anne sayısı hedeflenmemiş, annelerin görüş ve uygulamaları birbirini tekrarlayıp, değişik görüşler ortaya çıkmadığı izlenimi edinilene kadar çalışma sürdürülmüştür. Görüşmeler 7 aylık bir zaman diliminde tamamlanmıştır. GEREÇ VE YÖNTEM Annelerin, süt çocuğu beslenmesi konusundaki bilgileri ile davranışları, algı ve inanışlarından etkilenerek ortaya çıkan davranışlardır. Bu nedenle annelerin karar vermesini etkileyen süreci anlamayı amaçladığımız bu çalışmada araştırma tipinin niteliksel olmasına ve verinin birebir derinlemesine görüşmeler yoluyla toplanmasına karar verilmiştir. Annelerin yaşı, eğitimi, mesleği, eşinin mesleği ve işi, çocuk sayısı sorulmuştur. Bebeklerin boy, ağırlık ve yaşları kaydedilmiş ve Gomez sınıflamasına göre (yaşa-göre-ağırlık) beslenme durumları değerlendirilmiştir. Çalışma grubu, yaş, eğitim, ve ekonomik düzeyleri farklı olan annelerden seçilerek bu değişkenler nedeniyle yapılan farklı uygulamalar varsa onlara rastlama olasılığını artırmak 14 Marmara Medical Journal 2006;19(1);13-20 Gizem Samlı ve ark. Ortaya çıkan ana temaların dışında kalsa, ya da tek bir anne tarafından ifade edilmiş olsa bile tüm cümleler dikkate alınmıştır. Bu bilgiler ışığında annelere bebek beslenmesini iyileştirme konusunda sunulacak öneriler belirlenmiştir. ‘’İlk 5 gün sütüm yoktu, tanıdık kadınlar gelip emzirdi. Şerbet de verdik. Sonra sütüm geldi’’. ‘’Mememin ucu yoktu. Kaba sağdım. Kabın temizliğinden emin olamayınca mamaya başladım üçüncü ayda. İdrar yolu enfeksiyonu geçirince kaptan mikrop kaptırdığımı düşündüm.’’ ‘’Ağladıkça emziriyorum’’ ‘’İlk günler çabuk yoruluyor diye sık sık emzirdim’’ ‘’Bir buçuk yaşına kadar emzirmeyi düşünüyorum.’’ ‘’Aile büyüklerimiz ek gıdaya erken başlamamızı istiyordu. Ancak ben elimdeki kitapçığa göre 6 ay boyunca emzirdim. Altıncı ayda başka şeylere başladım.’’ Araştırma planı Koacaeli Üniversitesi Tıp Fakültesi Etik Kurulu tarafından onaylanmıştır. BULGULAR Toplam 40 anneden oluşan çalışma grubunun demografik özellikleri ve bebeklerin beslenme durumu Tablo I’de gösterilmiştir. Analiz sonucunda ortaya çıkan ana temalar Tablo II’de sıralanmıştır. 3-Anne sütüyle ilgili bilgi ve inanışlar: Anne sütünün üstün nitelikleri oldukça fazla sayıda katılımcı tarafından farklı yanlarıyla ifade edilmiştir. Ancak kolostrumun bebeğe verilmemesi ile ilgili inanış da tekrar edilen ifadelerden biri olmuştur. 1-Annelerin anne sütü verme kararını etkileyen süreçler, kişiler, kaynaklar: Anneler, bebeklerini emzirme kararını verirken evdeki büyüklerin ve çevrenin yanısıra çeşitli basılı eğitim malzemeleri, ile basın yayın organları ya da sağlık çalışanları tarafından verilen bilgilerden etkilendiklerini belirttiler. ‘’Annem emzirilen bebek daha iyi olur dedi.” ‘’Anne sütünü 2 yaşa kadar vereceğim. Televizyonda öyle diyorlardı.’’ ‘’Doktor kontrolde anne sütü yeterli mamaya gerek yok dedi, ama evdeki baskılara dayanamadım. Biberonu zorla verdim. Sonra da anne sütünü iyice bıraktı.’’ ‘’Çevremden ‘’bebeğini aç bırakıyorsun’’ suçlamaları bile geldi ama ben ilk 6 ay anne sütü dışında bir şey vermedim.’’ ‘’Bana beslenme konusunda kitapçık verildi. Çalışınca herhalde stres ve yorgunluktan sütüm azaldı.’’ 4-Mama başlama kararını etkileyen durumlar: Annelerin mama başlama kararını nasıl verdikleri konusundaki ifadeleri sık uyanma, annenin ilaç kullanması, annenin hamile kalması, kayınvalidenin isteği, mamanın daha besleyici olduğu şeklindedir. 2-Emzirme uygulamaları: Annelerin nasıl ve ne kadar emzirdiklerini belirleyen bazı geleneksel uygulamalar olduğu gibi sağlık sektörü tarafından hazırlanmış basılı malzemelerin de izlendiği ve dikkate alındığı belirtilmiştir. 15 Marmara Medical Journal 2006;19(1);13-20 Gizem Samlı ve ark. Suyla pişiriyorum.’’ 5-Annelerin ek gıdalarla ilgili bilgi ve tutumları: Yöresel, çevresel ve ekonomik durumların ek gıdalarla ilgili bilgi ve tutumları etkilediği ancak annelerin yazılı ve görsel basın ile çocuğun tercihlerinden de etkilendikleri belirtilmiştir. Ekonomik koşullar önemli bir sınırlayıcı olarak tekrar edilmiştir. ‘’İki aydan sonra çocuğa inek sütü verebiliriz.’’ ‘’Doymayıp ağlayınca, ineğimiz olduğu için, başka mama alacak paramız olmadığı için inek sütü verdim.’’ ‘’Mısır unu, buğday unu ve tereyağı karıştırıp kaynatıp kavanozlara koyuyoruz. Kaynanam laz maması diyor. Her öğünde bir kaşık sütle karıştırıp veriyoruz.’’ ‘’Televizyonda dinledim. Bazı anneler ise sağlık ocaklarında süt çocukları için beslenme kılavuzlarının olmadığından yakınmıştır. Annelerin çoğu anne sütüne en yakın besinin eczanelerde satılan hazır mama olduğunu ve sütleri yetmediğinde ilk 6 ayda mama vermek istediklerini belirtmiştir. İlk 6 ayda suyla pirinç unu, suyla Arı mama®, bazen de sulandırılmış inek sütü verdiğini ifade eden annelerin hemen hepsi ekonomik güçlüklerden de söz eden annelerdi. Bazı anneler inek sütünü erken kullanmanın doğru olmadığını, alerji ve demir eksikliği gelişmesi gibi sakıncaları olduğunu söylemiş ve çoğu anne 6 aydan sonra inek sütü yerine muhallebi ve yoğurt kullanmayı tercih ettiklerini belirtmişlerdir. Yoğurdun kemik gelişimi için önemli olduğu ve kolay hazmedileceği belirtilmiştir. Taze sebze ve meyvelerin yararlı olduğunu birçok anne belirtmiş, meyve tüketimi sırasında yine ekonomik güçlüklerden söz edilmiş, meyvelerin suyunu sıkmak veya püre hazırlamak yerine bir 17 Marmara Medical Journal 2006;19(1);13-20 Gizem Samlı ve ark. Annelerin emzirme ve süt çocuğu beslenmesi konusundaki bilgi, inanış ve uygulamaları: niteliksel bir araştırma yaş altındaki bebeklerinin eline bir parça meyve vermeyi tercih ettiklerini söylemişlerdir. doktor önerisiyle başlarken, az sayıda annenin doktor veya hemşireler tarafından verilen yanlış öneriler nedeniyle (annenin meme dışı lokal enfeksiyonu, antibiyotik kullanımı, uygunsuz kontraseptif seçimi) emzirmeye ara verdiği dikkat çekicidir. Ülkemizde bu konuda yapılan bazı çalışmalarda gözlemlerimizi destekler sonuçlar elde edilmiştir. Koçoğlu ve ark.’nın8 çalışmasında kayınvalidesi ile oturulan ailelerde bebek beslenmesinde söz sahibi olan kişinin daha çok kayınvalide olduğu ortaya çıkmıştır. Aynı çalışma anne sütünün ilk 6 ayda en yararlı besin olduğunun annelerin %91,2’si tarafından bilindiğini, ne kadar süre tek başına yeterli olduğunu ise annelerin ancak %58,8’inin bildiğini ortaya koymuştur. Potur ve ark.’nın9 çalışmasında, 1992 yılında Zeynep Kamil Hastanesi’ne başvuran 3 aylık bebeklerden ancak %15’inin sadece anne sütüyle beslendiği saptanmıştır. Erdöl ve ark.’nın10 Trabzon’da yaptıkları çalışmada da genellikle 3-4 ay civarında ek gıdaya başlandığı bildirilmiştir. Yücecan ve ark.’nın11 çalışmasında ilk verilen ek besini %21,9 oranında meyve suyu, %19,7 oranında hazır mamalar, %17,7 inek sütü, %13,4 muhallebi, %9,5 oranında yoğurt oluşturmuştur. Saner ve ark.’nın12 çalışmasında 3.ay sonunda emzirme oranı doğumdan hemen sonra ve 2-3. günde bilgi verilenlerde %95 oranında bulunurken, sadece bir kez görüşülen ile hiç görüşülmeyenlerde sırasıyla %50 ve %20’ye düşmüştür12. Çalışma birebir eğitimin önemini çarpıcı şekilde vurgulamaktadır. Annelerin bir kısmı sebze mamalarına et ilave etmediklerini, kemik suyunun bunun yerine geçtiğini söylerken, büyük bir kısmı da et alamadıklarını söylemişlerdir. Kahvaltıda yenen besinler pahalı olması nedeniyle annelerin çoğu yeterince peynir, süt, yumurta veremediklerini, bebeklerine çay-bebe bisküvisi, ekmek-çay-margarin, patates kızartması verdiklerini anlatmışlardır. Anneler, çocuklarının tercihleri konusundaki yargılarını “tadını beğenmiyor”, “abur cuburu seviyor”, “yemez”, “istemez” sözleriyle belirtmişlerdir. TARTIŞMA Annelerin eğitim ve sosyal düzeyleri ne olursa olsun hepsinin anne sütünün çocuklarının gelişimi ve hastalıklardan korunması için en yararlı besin olduğunu bildiği ve anne sütü vermeye başladığı görülmüştür. Zaten Türkiye’de emzirmenin yaygın olduğu ve tüm çocukların %97’sinin bir süre emzirilmiş olduğu bilinmektedir2,3. Ancak, esas endişe yaratan aşama, annelerin ne zaman mamaya başlanması gerektiği konusundaki kararlarıdır. Aile büyüklerinin ve yakınlarının bu konudaki önerileri, çoğu zaman bebeğin kilo alımını değerlendirmeden gereksiz ek gıdalara başlanarak bebeğin anne sütünden uzaklaşmasına neden olunduğunu düşündürmüştür. Eğitim düzeyi yüksek olan annelerin ve babanın da anne sütünün yeterliliği konusunda bilgilendirilmiş olmasının sadece anne sütüyle beslenilen süreyi uzattığını, ek gıdalara başlamaya neden olan sosyal baskıyı aşmada çevrenin de bu yönden cesaretlendirilmesi gerektiğini gösteren çalışmalar vardır6. Çalışmamızda çoğu anne, anne sütünün yanısıra su da verilmesi gerektiğini düşünmektedir. Çalışan annelerin 6 aydan önce ek gıda başlamalarında ülkemizde doğum sonrası iznin kısa olması etkilidir. Çalışan annelerin buna rağmen anne sütüne devam etme konusundaki çabaları dikkati çekmiştir. Amerika Birleşik Devletlerin’de anne sütü ile beslenmenin yüksek düzeyde eğitim almış, gelir düzeyi iyi, 30 yaşını aşmış kadınlarda en yüksek oranda olduğu rapor edilmiştir13. Carmichael ve ark.14 tarafından yapılan bir çalışmada da işe ya da okula erken dönmenin, 20 yaşından daha genç olmanın ve yöresel alışkanlıkların ek gıda başlamayı etkileyen durumlar olduğu gösterilmiştir. Yılmaz ve ark.’nın15 1999’da gerçekleştirdikleri çalışmada üniversite mezunu annelerin daha uzun süre anne sütü verdikleri görülmüştür. Amerikan Pediatri Akademisi ve Dünya Sağlık Örgütü (DSÖ) ilk 4-6 ay sadece anne sütü ile beslenmeyi, anne sütüne en az bir yaşına kadar devam edilmesini önermektedir16,17. 1989’da DSÖ ve Birleşmiş Milletler Çocuklara Yardım Fonu emzirmenin özendirilmesi ve desteklenmesi için doğum hastanelerindeki eğitim ve bebeklerin Annelerin ifadeleri özel doktor muayenehanelerine veya hastanelerin sağlam çocuk polikliniklerine gittikleri zaman beslenme konusunda sözlü olarak eğitildiklerini ve yazılı materyallerle ulaşabildiklerini, ancak gebelik takipleri ya da çocuklarını aşılama için sağlık ocağına götürmelerine rağmen bu sırada kendilerine anne sütü ve beslenme ile ilgili bilgilerden söz edilmediğini göstermiştir. Oysa annelerin anne sütü verme kararlarını, henüz antenatal kontrollerinde hekim ya da ebelerden aldıkları danışmanlık sırasında etkili olarak verdiklerini gösteren çalışmalar vardır7. Eğitimlerde anne sütünün 6 ay tek başına yeterli olduğu ve ek gıda başlamak için en önemli kriterin bebeğin yetersiz kilo alımı olduğu vurgulanmalıdır ve annelerin sütlerine güven duymaları amaçlanmalıdır. 7. Eğitimlerde ek gıdaların içerikleri, yararları, başlanmaları için uygun zaman belirtilmelidir. 8. Birebir eğitimlerde doktor ve hemşireler annenin olanaklarını gözeterek en uygulanabilir beslenme önerilerini vermeye özen göstermelidir. 9. Bebek beslenmesinde eğitimin yanı sıra ailedeki çocuk sayısının da etkisinin büyük olduğu inkar edilemez. Bu eğitimler sırasında özellikle düşük sosyoekonomik düzeydeki ailelerin daha az sayıda çocuk sahibi olmaları için aile planlaması konusunda yönlendirilmeleri gerekmektedir. Süt vermeye engel olmayacak yöntemler özendirilmelidir. 10. Anne sütü vermenin sakıncalı ya da olanaksız olduğu durumlarda bir seçenek olması nedeniyle, anne sütüne yaklaştırılmış mamaların ithal edilmesi yerine yurdumuzda üretilmesi ve daha ucuza sunulması sağlanmalıdır. 11. Satışa sunulan ve anne sütüne eşdeğer gösterilen mamaların içeriklerinin standartlara uygunluğu denetlenmeli, kutuların üzerinde anneleri yanıltıcı bilgilere yer verilmesi önlenmelidir. 12. Çalışan annelerin yeterli süre emzirebilmelerini sağlamak için doğum sonrası ücretli izin süresinin uzatılması sağlanmalıdır. 13. İşyerlerinde annenin sütünü sağması için uygun bir oda oluşturulması zorunlu hale getirilmelidir. anneleriyle aynı odada kalarak emzirilmelerine ilişkin prensipler ortaya koymuştur16. Karavuş ve ark.’nın18 çalışmasında 0-3 aylık çocukların %87’sinin, 4-6 aylıkların %74,2’sinin, 7-9 aylıkların %43,4’ünün ve 10-12 aylıkların % 44,8’inin anne sütü almaya devam ettikleri saptanmıştır. Ekonomik koşullar, taze meyve sebze tüketimi, formül mama kullanımı, yumurta, peynir ve ete başlanmasını etkileyen bir güçlük olarak tarif edilmiştir. Bu nedenle malnütrisyoninfeksiyon-süt çocuğu ölümleri kısır döngüsünün kırılması için ilk 6 ay sadece anne sütü verilmesinin sağlanması düşük ekonomik düzeydeki aileler için daha da önem kazanmaktadır. Annenin multipar olması, daha önce emzirme deneyiminin olması, aile ve arkadaş çevresinde emzirmenin yaygın olması ve babanın anneyi bu konuda desteklemesi ve annenin diğer konulardaki yükünü paylaşmasının sadece anne sütüyle emzirme sürecini uzattığını gösteren niteliksel çalışmalar vardır14,19. Anne eğitim düzeyinin yüksek olmasının daha az sayıda çocuk sahibi olma, ailede çocuk başına düşen gelirin daha yüksek olması, annenin çocuk beslenmesine daha çok zaman ayırması ve emzirmeye daha uzun süre devam etmesi ile ilişkili olduğu gösterilmiştir20. Dolayısıyla, annelerle her karşılaşma bir fırsat olarak kabul edilmeli ve eğitim amacıyla değerlendirilmelidir. Bebek beslenmesi konusunda annelerin düşünce ve uygulamalarının belirlenmesi, onların gereksinimlerini karşılayacak eğitim materyallerinin hazırlanması konusunda ilk adım olmalıdır. Bu çalışmanın verilerinden yola çıkarak bebek beslenmesinin iyileştirilmesi konusunda aşağıdaki öneriler sıralanmıştır. KAYNAKLAR 1. Clinical BPH refers to the presence of lower urinary tract symptoms (LUTS) and BPH in men. Clinical BPH is a common disease of the elderly male population with an incidence reaching nearly % 70 after the age of 60 years. Different studies have been performed to identify the risk factors for clinical BPH including age, obesity, hypertension, diabetes, vasectomy, sexual activity, physical activity, alcohol consumption, cigarette smoking, caffeine consumption, hormonal status and hyperlipidemia. However, the potential role of the afore-mentioned risk factors in the pathogenesis of clinical BPH is not clarified yet. In this study we investigated whether certain risk factors such as hypercholesterolemia, cigarette smoking and alcohol consumption play a role in the occurrence of clinical BPH. Methods: Between 1997 and 2000 a total of 142 patients admitted to the outpatient clinic with lower urinary tract symptoms (LUTS) were included in this prospective study. The International Prostate Symptom Score (IPSS) was administered to all patients by the primary physician and history of cigarette smoking and alcohol consumption was noted. On the day of examination, fasting blood samples were collected for serum cholesterol and PSA. All patients underwent uroflowmetric analysis. Patients with benign prostatic enlargement in digital rectal examination and with an IPSS>7 and a maximum flow rate < 10 ml/sec were considered as having clinical BPH. The r esults were analysed by student t test, Chi square test or ANOVA where appropriate. Results: Ninety-seven (%68.3) patients were found to have clinical BPH whereas 45 (31.7%) patients were not diagnosed as clinical BPH. The mean serum cholesterol level of the patients with and without clinical BPH was 226 mg/dl and 224 mg/dl, respectively (p>0.05). Thirty-four (35.1%) patients with clinical BPH were regular smokers whereas 25 (55.6%) patients without BPH were smokers (p=0.047). The ratio for alcohol consumption was 6% in clinical BPH patients and 13.3% of the patients without clinical BPH (p=0.041). According to PSA values, 23 (23.8%) patients who had clinical BPH had PSA>4ng/ml whereas 7 (15.6%) patients without clinical BPH had PSA>4ng/ml (p>0.05). Conclusion: Our results indicate a protective effect of cigarette smoking and alcohol consumption in the occurrence of clinical BPH whereas serum cholesterol and PSA levels did not reveal a significant effect on the occurrence of clinical BPH. ÖZET Amaç: Benin prostat hiperplazisi (BPH), histopatolojik bir terim olup, prostatın iyi huylu morfolojik büyümesi anlamına gelmektedir. Klinik BPH ise erkeklerde BPH ile birlikte ona bağlı olduğu düşünülen alt üriner sistem semptomlarının bulunması olarak tanımlanır. Klinik BPH, yaşlı erkek popülasyonunda oldukça sık karşılaşılan ve 60 yaş üzeri hastalarda % 70’lere varan insidansa sahip olan bir hastalıktır. BPH gelişiminde, risk faktörü olabileceği düşünülen, yaş, obesite, hipertansiyon, diyabet, vazektomi, seksüel aktivite, fiziksel aktivite, alkol kullanımı, kafein kullanımı, hormonal faktörler ve hiperlipidemi hakkında değişik çalışmalar yapılmıştır. Ancak, bu risk faktörlerinin klinik BPH patogenezindeki rolü henüz tam olarak aydınlatılamamıştır. Bu çalışmada, hiperkolesterolemi, sigara ve alkol kullanımı gibi risk faktörlerinin klinik BPH gelişimindeki önemi araştırılmıştır. Yöntem: Bu prospektif çalışmaya, 1997-2000 yılları arasında polikliniğimize alt üriner sistem şikayetleri ile başvuran toplam 142 hasta alındı. Her hastanın sigara ve alkol kullanma hikayeleri alınarak çalışmayı yürüten hekim tarafından yüz yüze görüşme yapılarak uluslararası prostat semptom skoru formu (IPSS) uygulandı. Muayene gününde ayrıca total kolesterol ve PSA analizi amacıyla hastalardan açlık kanı alındı ve ardından tüm hastalara üroflovmetrik analiz yapıldı. Parmakla rectal muayenede prostatta benin büyüme tespit edilen, IPSS>7 ve maksimum akış hızı <10 ml/sn. olan hastalara klinik BPH tanısı konuldu. Çalışmada tespit edilen sonuçlar student t testi, ki-kare testi ve ANOVA ile analiz edildi. Bulgular: Hastaların 97’sinde (%68.3) klinik BPH tespit edilirken 45 (%31.7) hastanın bulgularının klinik BPH ile uyumlu olmadığı gözlendi. Klinik BPH’sı olan hastaların ortalama serum kolesterol seviyesi 226 mg/dl iken, klinik BPH’sı olmayan hastalarda 224 mg/dl olarak tespit edildi (p>0.05). Klinik BPH tanısı alan hastaların 34’ü (%35.1) sigara kullanırken 63 (%64.9) hasta sigara kullanmamaktaydı (p=0.047). Alkol kullanımı sorgulandığında klinik BPH’sı olan hastaların % 6’sı alkol kullanırken, klinik BPH’sı olmayan hastaların %13.3’ü alkol kullanmaktaydı (p=0.041). PSA değerlerine bakıldığında, klinik BPH tespit edilen hastaların 23’ünde (%23.8) PSA>4ng/ml iken klinik BPH’sı olmayan hastaların 7’sinde (%15.6) PSA>4ng/ml olarak tespit edildi (p>0.05). Sonuç: Çalışma grubumuzda sigara veya alkol kullanan hastalarda klinik BPH görülme sıklığı anlamlı olarak düşük tespit edildiğinden, bu çalışmada sigara ve alkol kullanımının klinik BPH gelişiminde koruyucu etkiye sahip olabileceği düşünüldü. Serum kolesterol ve PSA seviyelerinin ise, klinik BPH gelişiminde anlamlı bir etkiye sahip olmadığı gözlendi. The incidence of BPH is also low in vegetarian men4. The reason for this finding may be related with diet since Asian and vegetarian men have low-fat and high fibre diets which lower the blood cholesterol level and provide pyto-estrogens that are proposed to be preventive against BPH. It is also notable that the lower incidence of clinical BPH in Asian men increases in immigrant generations after they have started to live in North America.4. This phenomenon is thought to be due to a high fat diet, since in another study it was shown that there is a relation between a high fat diet and BPH5. INTRODUCTION BPH is a common disease and a major cause of morbidity in elderly men which may lead to bladder outflow obstruction and LUTS. Nearly 70 % 60-70 year- old men have LUTS related to BPH and 25-30% of the men have had surgical treatment by the age of 80 years1. So far, two factors are established which are mandatory for the occurrence of BPH: Age and a testis with normal testosterone production2. Several studies have been performed to identify additional risk factors, especially for clinical BPH including age, obesity, hypertension, diabetes, vasectomy, sexual activity, physical activity, alcohol consumption, cigarette smoking, caffeine consumption, hormonal status and hyperlipidemia. However, any additional strong risk factors have yet to be identified. There are also conflicting results concerning the effect of cigarette smoking and alcohol consumption on BPH. Although cigarette smoking and moderate alcohol consumption was found to be protective against BPH, some studies showed 22 Marmara Medical Journal 2006;19(1);21-26 Tufan Tarcan, et al. As illustrated in Table I, the severity of the symptoms increased with the increasing age (student’s t test, p=0.021), but there was no statistically significant relation between the age and Q max value of the patients (student’s t test, p>0.05). that these extrinsic factors had no effect on BPH6,7. The aim of our study was to evaluate the relation between hypercholesterolemia, cigarette smoking, alcohol consumption and clinical BPH. METHODS Between 1997 and 2000 a total of 142 patients over 45 years of age admitted to our outpatient clinic with LUTS were included in this prospective study. All patients underwent routine urological assessment including history, physical examination, urine analysis and cultures, and serum PSA levels. Patients with suspected prostate cancer after digital rectal examination and/or a PSA> 4 ng/ml were sent to prostate biopsy. Patients with urinary tract infections and/or other significant urological conditions such as, urolithiasis, prostate or bladder cancer were excluded from the study. The IPSS was administered to all patients by the primary physician and histories of cigarette smoking and alcohol consumption were noted. We defined clinical BPH as patients who have an IPSS >7 with an enlarged prostate detected in digital rectal examination and found to have a maximum flow rate (Q max) <10 ml/sec in uroflowmetric studies. Smokers were defined as the patients who regularly smoke one or more cigarettes per day. Alcohol consumers were defined as patients who drank one or more glasses of alcohol more than 2 days per week. On the day of examination, fasting blood samples were collected for serum cholesterol and PSA measurements and then, all patients underwent uroflowmetric studies. The statistical analysis was performed with unpaired student t-test, Chi square test or ANOVA test where appropriate. However, when patients with clinical BPH were considered we only found a statistically significant difference between patients <50 and >50 years of age in terms of IPSS (student’s t test, p<0.01) whereas there was no significant difference in other age groups in terms of IPSS and there was no significant difference in terms of Q max values in all age groups (student’s t test, p>0.05) (Table II). The mean serum cholesterol level of the patients who did not have clinical BPH was 224 mg/dl and in comparison similar to 226 mg/dl found in patients with clinical BPH. (ANOVA, p>0.05). We grouped the patients according to serum total cholesterol level into 3 groups: In group 1, there were patients with normal total cholesterol level (<200mg/dl), in group 2, there were patients with mild hipercholesterolemia (200mg/dl-250mg/dl) and in group 3 there were patients with moderatesevere hipercholesterolemia (>250mg/dl). There was no statistically significant difference between the 3 groups in terms of IPSS, Qmax values and the ratios of the clinically BPH patients, (ANOVA, p>0.05) (Table III). When patients with clinical BPH were analysed in terms of smoking we found that 65% (13/20) patients in the age range between 40 and 59 years were smokers versus 27% (21/77) of patients aged 60 and older (p<0.05) whereas for patients without clinical BPH In patients with clinical BPH, 34 (35.1%) patients were smokers whereas 63 (64.9%) patients were non-smoker. In contrary, in patients without clinical BPH, the majority (n=25, 55.6%) of the patients were smokers versus 20 patients (44.4 %) who were non-smokers. There was a statistically significant difference between patients with and without clinical BPH in terms of the incidence of smoking (chi square, p=0.047) (Table IV). The ratio of alcohol consuming patients was more than twice as high in the presence of clinical BPH when compared to patients without clinical BPH (6.2 % and 13.3% patients with and without clinical BPH, respectively (chi square, p=0.027) (Table IV). We also compared the relation between smoking and alcohol consumption according to the age groups of the patients.(Table V) RESULTS The mean age of the whole study group was 67.3 years (45-89 years) and the mean PSA value was 2.94 ng/ml (0.1-15.1 ng/ml). Patient’s IPSS ranged between 2-35 (mean: 19.3), Q max between 2-28 ml/sec (mean 9.4 ml/sec) and total cholesterol 82-461 mg/dl (mean: 226 mg/dl). Overall, 54 (38%) and 88 (62%) of the patients were smokers and non-smokers, respectively, whereas only 12 (8.4%) patients were alcohol consumers as defined previously. In the whole study group, 97 (68.3%) patients were found to have clinical BPH whereas 45 (31.7%) patients did not. The mean age of the patients with and without clinical BPH was 67.3 (50-78 years) and 66.9 (45-89) years, respectively. DISCUSSION BPH is a common cause of morbidity in elderly patients and may be defined as a biological response of prostate to aging. The direct relation between BPH and aging was documented in different series 2,8. The present study has also revealed a direct relation between age and the incidence of clinical BPH. Thus, the aging process is one of the most important risk factors for BPH. Another well-known risk factor for BPH is the presence of androgens in the circulation. It has been illustrated that BPH does not occur in men who were castrated before puberty and is very rare in men who were castrated before 40 years2 of age. Another risk factor evaluated in this study was alcohol consumption. Gordon et al reported that high levels of alcohol consumption cause a decrease production and increase the metabolism of testosterone 11. Morrison proposed that daily beer intake lowered the risk of clinical BPH and in this study; the relative risk of clinical BPH in alcohol consumers was calculated as 0.618. In another study, Gass et al evaluated a total of 882 men aged between 65-80 years and reported that patients who had regular alcohol intake had % 20 less clinical BPH 12. These results were similar to our study in that we also found a statistically significant protective effect of alcohol consumption against clinical BPH (p=0.041). Therefore, it is logical to investigate the potential role of many other extrinsic or intrinsic risk factors that may cause abnormality in hormonal status such as cigarette smoking, alcohol consumption, obesity, hypercholesterolemia, physical activity. For example, one of the factors that affect the hormonal status is cigarette smoking. Field et al, demonstrated that cigarette smoking increases the dehydrotestosterone level which stimulates the prostate gland and proposed that this may increase the risk of BPH 9. But, Matzkin et al, reported no difference in prostate volume among smoker and non-smoker patients 6. On the other hand, some epidemiological studies have found evidence of a lower risk of BPH in cigarette smokers 2,6,7. Meigs et al followed 1709 men aged between 40-70 years for 9 years and observed that men who smoked had %50 less clinical BPH during the follow-up period 2. Although Seitter et al found no relation between smoking and BPH 10, there is growing evidence in the literature for a weak protective effect of cigarette smoking in the occurrence of BPH. In our study, we also found a statistically significant protective effect of cigarette smoking on clinical BPH (p=0.047). This protective effect can be It is an interesting finding that BPH incidence is lower in Asian countries than in Western countries whereas Asian immigrants in the United States have the same incidence of clinical BPH as their white American counterparts4. Dietary factors seem to explain this phenomenon since Asian people consume more low-fat, high-fiber diets than Western people. In different series, it was shown that high energy and animal product diets increase the risk of BPH while fruit and vegetable based diets have a protective effect against BPH13. Lagiou et al dictated that there is a positive association of BPH risk with butter, margarine and seed oils which can increase serum cholesterol levels13. It was shown that high fat diet causes an elevation at plasma testosterone level and might be associated with BPH5. In a prospective follow-up study, Meigs et al reported a double risk of BPH in patients with coronary heart disease2. Kitagawa et al showed a correlation between serum fat content and prostatic growth so they suggest that there may be an association between BPH and abnormal lipid metabolism14. Hammarsten et al examined 158 patients with BPH and found a statistically significant relation between BPH and low HDL/cholesterol ratio15. Indeed we could not find a statistically significant relation between total cholesterol level and clinical BPH (p>0.05). 5. 6. 7. 8. In our study, we found that cigarette smoking and alcohol consumption appear to have significant protective effects against clinical BPH. The pathophysiological mechanism behind this finding remains to be clarified by further studies. The present study did not reveal a significant relation between total cholesterol or PSA levels and the presence of clinical BPH. Patients and Method: Six children (three boys, three girls, mean age 11.5 years) with the diagnosis of chorea were treated with carbamazepine (5-20 mg/kg/day). Response to the treatment was assessed based on improvement of hand-writing and Archimedes spiral test. Results: Clinical improvement was observed in all children within the first two weeks. Chorea totally disappeared in four patients. In the other two patients chorea did not disappear totally, however markedly improvement was observed. No side effects were seen. Conclusion: Carbamazepine is an efficient and safe drug in the treatment of chorea in children. Dört hastada kore tamamen kaybolurken, iki hastada belirgin iyileşme görüldü. Yan etki saptanmadı. Sonuç: Karbamazepin çocuklarda kore tedavisinde etkili ve güvenilir bulunmuştur. Anahtar Kelimeler: Kore, Karbamazepin, İstemsiz hareket, Çocuk INTRODUCTION METHODS Chorea is an involuntary movement, sometimes difficult to treat, resulting from more than a hundred causes and Sydenham chorea is the most frequent form of acquired chorea during childhood1. Although multiple drugs have been used in the treatment of chorea in children, no adequate data is available based on randomized, controlled studies about their doses, effects as well as the duration of treatment2-5. Carbamazepine (CBZ) has been used in the treatment of movement disorders since 1969, however its effective dose and mechanism remain unclear6. Clinical findings of six patients with chorea taking carbamazepine therapy were reported and the results of published reports were evaluated. Six patients (three boys, three girls) between the ages of 8-15 (mean 11.5 years) were diagnosed as having chorea and treated by CBZ between September 2000 and June 2005. The etiologies of chorea were as follows: Sydenham chorea (n: 4), dyskinetic cerebral palsy (n:1), basal ganglia infarction due to antiphospholipid syndrome (n:1). The diagnosis of “Acute Rhematic Fever” was made on the basis of modified Jones criteria and all patients with Sydenham chorea had carditis. All of the children presented hemichorea except for the patient with dyskinetic syndrome. CBZ was started in 5mg/ kg/ day and the daily dose was increased until the involuntary movements seemed to decrease or disappear. Response to treatment was assessed based on improvement of hand-writing and Archimed spiral test as being mild or moderate improvement and complete remission. RESULTS Table I shows clinical findings and the patient responses to treatment. Clinical improvement was observed within the first two weeks of treatment in all patients and no side effects were seen. Chorea totally disappeared in four patients with Sydenham chorea, and the treatment was sustained between 7 – 14 months (mean 11.5). In the other two patients, chorea did not disappear totally but markedly improved and the patients’ daily life quality concerning their capacity of using required motor fuction abilities (fine motor function) was better. of our four patients with Sydenham chorea were treated longer than 14 months. However, our two patients with permanent chorea are still under treatment for 48 months. Knowledge in the literature about the duration of treatment in children with different etiology is still insufficient. Based on the results of previous studies and our own results, we conclude that carbamazepine (420mg /kg /day) is an efficient and safe drug in the treatment of chorea. Because of the wide range of effective dose, CBZ should be started clinically in minimum effective doses and the dose may be increased until a sufficient clinical response is achieved. DISCUSSION Neuroleptics (haloperidol, risperidone, fluphenazine) and antiepileptics (phenobarbital, clonazepam, valproate, carbamazepine) have often been used in the treatment of chorea. There are few studies regarding the clinical effects of these drugs, minimum effective doses, and therapeutic blood levels in childhood2-5. Table II shows the results of studies in the literature, demonstrating the effect of CBZ. Harel et al. reported that clinical improvement occurred in children who were treated with low dose of CBZ (4-10 mg/kg/days) and blood levels of CBZ were under the required level for the treatment of epilepsy2. CBZ and valproate have been found equally effective and safe drugs in the treatment of Sydenham chorea3,5. The effective mechanism of carbamazepine to the basal ganglia is not obvious, but it is postulated that it could be through the blockage of dopaminergic postsynaptic receptors and through the stimulation of cholinergic pathways4. Most of the reported children treated with CBZ were diagnosed as Sydenham chorea, and the duration of treatment in these patients was reported as lasting 1-15 months. Yapılan fizik incelemede göğüs, sırt ve sağ kolda yer yer küçük ekimotik alanlar ile sol üst kadranda palpasyonla hassasiyet saptandı. laboratuvar incelemelerinde kreatin fosfokinaz, kreatinin ve potasyum düzeyleri yüksek bulundu. Hasta, rabdomiyolize bağlı akut tubuler nekroz tanısıyla acil servise yatırıldı. İzotonik NaCl, sodyum bikarbonat ve furosemid tedavisi ile yatışının dördüncü gününde serum değerlerinin normal sınırlara gerilediği gözlendi. Travmada etkilenen kas kitlesinden bağımsız olarak serumda kas yıkım ürünlerinin oldukça yüksek bulunması akut tubuler nekroz olasılığını da beraberinde getirmektedir. Travma nedeniyle acil servise başvuran hastalarda etkilenen kas kitlesine bakılmaksızın serum CPK, Cr ve K+ değerleri incelenmeli, yüksek bulunan olgularda oluşabilecek komplikasyonların önlenebilmesi için tedaviye erken başlanmalıdır. We learned he had been in a fight, assaulted with fists and iron bricks and wounded in his abdomen, thorax and right arm. Clinical examination revealed echymotic areas in his thorax, back and right arm; tenderness in the left upper quadrant of abdomen. In laboratuvary tests creatinin phosphokinase (CPK), creatinin(Cr) and potassium(K+) levels were high. The patient was admitted to emergency service with a diagnosis of rhabdomyolysis. We started izotonic saline infusion, sodium bicarbonate and furosemide treatment. His serum levels returned to normal values on the fourth day of his admission. In a trauma patient independent of extent of muscle damage if enzyme levels are high; a probability of acute tubuler necrosis risk is also high. In patients presented to emergency clinic due to trauma, without looking at the extent of muscle injury ; CPK, Cr, K levels must be checked, and if they are high , to prevent complications treatment must be started immediately to prevent complications. Bu nedenle tedavide metabolik asidozun önlenmesi için diyette protein kısıtlaması (0,6 g/kg/g) ve alkali tedavi uygulanmaktadır. Bu hastaların idrarında görülen miyoglobin idrarın koyu renginden sorumludur5. OLGU SUNUMU 30 yaşında erkek hasta, acil servise sol üst kadran ağrısı ile başvurdu. Özgeçmişinde belirgin bir özelliği olmayan hastanın öyküsünden, bir gün önce bir sokak kavgasına karıştığı, tekme ve demir sopalarla göğsüne, sırtına ve sağ koluna darp edildiği öğrenildi. Başvuru sırasında bilinci açık, koopere ve oryante idi. Vital bulguları stabil olarak saptandı. Fizik incelemesinde sağ hemitoraks, sağ kol ve ön kol üzerinde 2-3 cm çapında, sol flank bölgesinde 3-4 cm çapında birkaç ekimotik alan saptandı. Dalak ve böbrek travması riski düşünülerek kan ve radyolojik incelemeler yapılması planlandı. Laboratuvar sonuçlarında serum kreatinin (Cr) 4.08 mg/dl (Normal: 0,5-.1,1) kreatin fosfokinaz (CPK) 985 U/l (Normal: 0-190), potasyum (K+) 6,3 mEq/L (Normal: 3,5-5,3), idrar incelemesinde makroskopik olarak hematürik görüntü vermesine rağmen mikroskopik incelemede eritrosit saptanmadı, pH 5.0 idi. Yapılan batın USG normal olarak değerlendirildi. Hasta travmaya bağlı rabdomiyoliz tanısıyla acil servise yatırıldı ve nefroloji konsültasyonu istendi. İzotonik NaCl 1,5 L/saat ardından 2,5 ml/kg/saat infüzyon; idrar pH alkali sınırlarda kalacak şekilde (>6.5) NaHCO3 (132 mEq/gün intravenöz infüzyon) ve zorlu diürez için furosemid (100 mg/gün intravenöz infüzyon) tedavisine başlandı. Hastanın sıvı dengesi, serum elektrolitleri, serum Cr ve idrar pH izlendi, tedavinin dördüncü gününde serum Cr 1,4 mg/dl, CPK 210 U/l, K+ 4,8 mEq/L gerilediği izlendi. Hasta nefroloji polikliniği takiplerine gelmek üzere sorunsuz olarak taburcu edildi. Son yıllardaki çalışmalar RML sonrası akut böbrek yetmezliği (ABY) riskinin % 0-67 olduğunu göstermektedir. Bu risk için belirleyici sebepler araştırıldığında etkilenen kas kitlesi yüzdesi ile bağlantı saptanamamıştır6. RML saptanan hastaların yakınma ve bulguları farklılıklar göstermektedir. Geniş bir çalışmada, RML hastalarının % 50’den fazlasında kas ağrısı ve kas güçsüzlüklerinin olduğu gösterilmiştir7. Bu da RML tanısının öykü ve fizik bakı ile dışlanmaması gerektiğini, laboratuvar testlerinin gerekliliğini göstermektedir. Hastanın genç ve sadece lokalize (sol üst kadran) ağrısının olması; öyküde koyu renkli idrardan bahsetmemesi RML tanısının atlanması ve belki daha sonra ABY gibi dramatik bir sonuç doğurabilirdi. Acil serviste travmalı hastalarda CPK, serum Cr ve K+ rutin testler arasında yer almamaktadır. Ancak bu ve benzeri durumlarda kas enzimleri ve hücre yıkımına bağlı CPK ve K+ yüksekliği akla gelmelidir. RML ve buna bağlı CPK yüksekliği için, travmaya maruz kalan kas kitlesi önemli olmasına rağmen; bu hastalarda ATN gelişmesi açısından tam olarak prediktif bir değer yoktur1. Yeni yapılan bir çalışmada da eksersiz sonrası artan serum CPK seviyesinin yüksekliği ile böbrek fonksiyon bozukluğu ve kas hasarının şiddetinin belirlenmesi arasındaki ilişkiye bakılmıştır. Ama belirli bir düzeyin üzeri için bir sınır değer (cutoff değeri) saptanamamıştır8. Dönmez ve arkadaşlarının çalışmasında, 1999 Marmara depreminde Crush Sendromu gelişen 20 hastanın % 55’inde yalnızca tek ekstremite hasarına bağlı da serum CPK, Cr ve K+ değerlerinin yüksek bulunduğu bildirilmiştir9. Bizim hastamızda göğüs bölgesinin yanı sıra sağ kola travma mevcuttu. Her iki bölgede kas kitlesi göreceli olarak az olsa da serum CPK ve K+ değerleri yüksek bulunmuştur. Travmaya uğrayan kas kitlesinden bağımsız olarak serum CPK ve K+ değerleri (dehidratasyon, alkol vb. hasta faktörleri ile birleşerek) ATN oluşturabilecek düzeylere ulaşabilir. TARTIŞMA Rabdomiyoliz, ilk olarak 1881 yılında Fleisher tarafından, kas egzersizleri ile birlikte hemoglobinüri görülmesi olarak tanımlandı1. 1941 yılında Bywaters ve Beall iskelet kası ezilme hasarı (Crush) ile akut tubuler nekroz (ATN) birlikteliğini ortaya koymuşlardır3. ATN’un en sık iki nedeni iskemi ve toksinlerdir. Miyoglobin, “hem” içeren ve ATN’a en sık neden olan nefrotoksindir. Nekrotik kas hücrelerinden salınan büyük miktardaki miyoglobin glomerüllerden serbestçe filtre olur ve renal tubullerden reabsorbe olarak direkt hasara yol açar. Pigment ayrıca, distal renal tubuler obstrüksiyon ile hasar oluşturabilir. Rabdomiyoliz genelde sağlıklı bireylerde travma, aşırı fiziksel aktivite, epileptik nöbetler, alkol ve başka ilaç alımları, enfeksiyonlar sonrası görülür. Miyoglobinürik akut böbrek yetmezliği sadece miyoglobin varlığında, hipovolemi ile renal hipoperfüzyon durumunda ortaya çıkar4. He had been investigated in a number of hospitals and had received multiple blood transfusions for iron deficiency anemia. On his first admission he was misdiagnosed as having gastrointestinal bleeding because of hematemesis and occult blood in the feces. Six months later, a history of coughing blood guided us to the probability of pulmonary hemosiderosis. The diagnosis of IPH was made after a lung biopsy showed a number of hemosiderosin-filled macrophages in the alveoli. Anahtar Kelimeler: Hemosiderosis, Demir, Hemoptizi, Çocukluk çağı INTRODUCTION CASE REPORT Idiopathic pulmonary hemosiderosis (IPH) is a rare disorder of unknown etiology characterized by iron deficiency anemia, recurrent or chronic pulmonary symptoms such as cough and hemoptysis and diffuse pulmonary infiltrates1. The incidence is reported from 0.24 to 1.23 cases per million in selected populations2. The clinical course of IPH is exceedingly variable, anemia, coughing and radiological evidence of pulmonary infiltrates are seen in most of the affected individuals but in the absence of pulmonary problems diagnosis and treatment are delayed. We reported a case with no pulmonary signs and with a misleading presentation of iron deficiency anemia, due to hematemesis and occult blood loss in the feces. A 12 year old boy was admitted to our hospital suffering from fatigue, pallor, anorexia and blood in his vomit. There was no history of hemoptysis or dyspnea. He had been investigated at a number of other hospitals and diagnosed as having iron deficiency anemia. The boy had received multiple blood transfusions and iron preparations. Physical examination revealed a child with extreme pallor, normal respiratory rate, a normal precordium with a grade 2/6 systolic ejection murmur, normal pulmonary auscultation. The liver and spleen were not enlarged. His weight and height were normal for his age. No abnormalities indicating hemolysis, chronic infection, malignancies, marrow dysfunction, hemoglobinopathies or coagulation disorders were found. Because of the finding of positive occult blood in feces and the history of hematemesis, a gastroduodenoscopy was performed and antral gastritis and erosive duodenitis was found. Helicobacter pylori was cultured and shown on biopsy. This led to a conclusion of iron deficiency most likely due to blood loss. The patient received blood transfusion and medical treatment for the helicobacter pylori. Therapy with prednisolone (2 mg/kg/day) was initiated. The patient’s clinical course improved after starting treatment and he was followed up. DISCUSSION IPH is a rare disorder characterized by recurrent or chronic diffuse alveolar hemorrhage and accumulation of hemosiderin in the lungs. The anemia caused by blood loss is enforced by the iron deficiency and presents as a microcytic anemia. Individuals of any age can be affected and it occurs with equal frequency in males and females3,4. Although its etiology remains unknown, IPH is considered to be an immune mediated disease1. The presence of ANCA is thought to be a sign of poor diagnosis for pulmonary progression. The presence of other antibodies seems to be the predictor for development of an immune response. Several cases were reported with iron deficiency anemia, pulmonary symptoms including cough, dyspnea and hemoptysis, diffuse paranchymal infiltrates on chest radiographs2,5. Diagnosis can be delayed in patients without pulmonary symptoms6,7,8. We had a six month delay before diagnosis and treatment because our patient had no respiratory findings on his first admission. The initial presentation with iron deficiency anemia, hematemesis, occult blood loss and erosive gastritis was misleading. Kipper et al3 noted there was a long delay (4 months- 10 years) between the starting of symptoms and the time of correct diagnosis. Six months later we saw him again with anemia, fatigue and pallor. Severe microcytic anemia was again present. The laboratory findings were as follows; Hb 3.6 gr/dl, Hct %12.9, MCV 66.5 fl, RDW:24.3, serum iron 12 mg /dl, serum iron binding capacity 344 mg /dl, and ferritin 133 mg /ml. At this time the boy told us that he had seen some blood after coughing. We considered the probability of pulmonary hemosiderosis. A computerized tomography (CT) scan of the thorax showed diffuse alveolar interstitial infiltration (Figs. 1a- 1b). Gastric aspiration and BAL for macrophages containing hemosiderin remained negative, so biopsy was performed and pulmonary hemosiderosis was diagnosed. For differential diagnosis of idiopathic pulmonary hemosiderosis antinuclear antibody (ANA), antineutrophil cytoplasm antibodies (ANCA), RAST for cow’s milk were checked and were found negative. The levels of complement C3 and C4 were normal. The diagnosis can be confirmed by demonstration of hemosiderosin-laden macrophages in the gastric aspirates or BAL. Studies indicate that patients show varied response to immunosuppressants. Early active and extended courses of immunosuppressive therapy (prednisone, hydrocloroquine, azothıoprıne, cyclophosphamide) may improve the prognosis. Saeed et al2 found that 5 year survival of patients with IPH was 86% because of long term therapy with immunosupressants. Treatment is continued according to patient’s needs. We concluded that IPH should be considered when investigating a possible cause of iron deficiency anemia especially in patients who require multiple blood transfusions. Many transfusion reactions are not recognized as such, perhaps because signs and symptoms mimic other clinical conditions. TA-GVHD is a rare but dangerous and fatal complication of blood transfusion. No costovertebral or suprapubic tenderness was detected. Both hips were mobile; no pain or limitation of motion was elicited. His hands and feet seemed edematous and erythematous (Figs. 3-5). Initial neurologic examination revealed no abnormalities except confusion. CASE REPORT A 58-year-old male patient who initially presented with fever, macular erythematous rash and pancytopenia was admitted to our clinic. He had no complaints until 2 days before admission when he noticed maculopapular rash on the dorsal aspects of his feet and over his chest. Later on, the rash spread to his arms and back (Fig. 1). The day before admission, he developed high fever (39.5ºC) and diarrhea. After the operation, 3 units of nonirradiated whole blood (one unit from his son, one unit from his cousin and another from an unrelated person) were transfused. His total blood count before the operation were within normal ranges [ White blood cells (WBC): 7300 / mm³, Hemoglobin (Hb): 14.9 mg/dL , Hematocrit (Htc): 33.8%, Platelets (Plt): 187000 / mm³]. He had used nonsteroidal anti-inflammatory drugs (ibuprofen, naproxen) and analgesics (paracetamol) for pain in the right hip joint. Cefazoline was given for surgical prophylaxis. Medical history of his family was unremarkable. Fig. 3: Diffuse erythema and edema of his hands and rash on forearms On admission, his temperature was 39.5ºC, pulse rate 88 / min, respiratory rate 20 / min and blood pressure 120/60 mmHg. On physical examination, the patient seemed fatigued and confused, erythematous macular rash was present on his abdomen, back, arms and purpuric rash with no bleach on pressure on his legs (Fig. 2). No lymphadenopathy was found. Hyperemia was observed on the pharynx and tonsils. The tonsils were also hypertrophic. Enanthems were observed on the hard palate. Based upon the persistence of fever after 48 hours, amikacin (1x1gr) was added to the therapy regimen. At the same time, prednisolone 1 mg/kg/day was started. In spite of this therapy, the fever persisted (40ºC maximally). Thus, ceftazidime plus amikacin therapy was switched to meropenem 3x1 gr/day IV. On the 10th day, the patient developed sore throat and dysphagia suggesting fungal esophagitis. Fluconazole 200 mg IV daily was started. Total blood count measurement was performed daily. Irradiated and filtered platelets and erythrocytes were transfused accordingly. No underlying immunodeficiency was identified. Fig, 5: Rash and edema of his feet The diagnostic procedure was a punch biopsy of the skin from the lesions on his lower leg. The specimen stained with hematoxylin and eosin was examined histopathologically under the light microscope. Hyperkeratosis and parakeratosis within the epidermis, spongiosis in the spinal layer cells and focal vacuolisation within the basal cell layer were seen. There was lymphocyte infiltration within the papillary dermis. Lmphocytes were also seen in the epidermis (exocytosis). Taking the clinical findings into consideration, the histopathological findings could be consistent with grade I-II of acute phase of GVHD2. Initial complete blood count revealed pancytopenia ( WBC: 700 /mm3, Hb: 8.5 mg/dL, Htc: 24%, Plt: 138000/mm3.) Erythrocyte sedimentation rate (ESR) was 139 mm/hr and Creactive protein (CRP) was 189 mg/L . Biochemical investigation of the patient’s blood revealed AST: 70 U/L , ALT: 123 U/L, ALP: 321 U/L, GGT: 285 U/L, total protein: 5 g/dL, albumin: 2.6 g/dL, globulin: 2.4 g/dL, total bilirubin: 1.7 mg/dL, glucose: 159 mg/dL, urea:41 mg/dL, creatinine: 0.8 mg/dL, uric acid: 2.3 mg/dL, sodium: 131 mEq/L, potassium: 3.6mEq/L, iron: 104 µg/dL, total iron binding capacity: 127µg/dL, prothrombin time: 16 sec, prothrombin activity: 69 %, INR: 1.4, aPTT: 29 sec. Bone marrow aspiration and biopsy were performed on the 9th hospital day. Erythrophagocytosis and hypocellular bone marrow with histiocytic reaction characterized by loss in the three series were reported. These findings could be consistent with immune system diseases, viral infections and the use of some drugs (e.g: chloramphenicol, phenylbutazone, anticonvulsants, sulfonamides etc.). During follow-up, his complete blood count values progressively decreased. Despite intensive followup and therapy, the patient died on the 13th hospital day (that is nearly five weeks after the implicated transfusion). Autopsy was planned but could not be carried out as the family members refused permission. On a peripheral blood smear, the lymphocytes were 97%, monocytes were 3%, no leukocytes, eosinophils, basophils, blasts or atypical cells were seen. The urine test was 2 positive (++) for bilirubin; the sediment contained 5-6 erythrocytes per high power field and no bacteria or casts. Radiographs of the chest were unremarkable. Ultrasonography of the abdomen revealed no abnormalities. Normal bacterial flora was isolated from the throat culture. Urine and blood cultures remained sterile. DISCUSSION Serologic tests for hepatitis markers (HbsAg, antiHBc IgM, anti-HCV, anti-HAV IgM), anti-HIV, anti-HSV 1-2 IgM, anti-CMV IgM, antiParvovirus B19 IgM were all negative. Serologic tests for Brucella (STA= standard tube agglutination) and monotest were also negative. Most cellular blood products, including red cell, platelet, and granulocyte products, contain viable, immunocompetent T lymphocytes 3. When transfused into immuno-incompetent recipients, these donor lymphocytes may proliferate in the patient and lead to the clinical syndrome of TAGVHD4,5. Leukopenia followed by pancytopenia due to marrow failure is quite common in TA-GVHD and is seen most often 2 to 3 weeks after the onset of symptoms. Diagnosis of TA-GVHD is difficult, and it is based on the clinical picture and can be confirmed histologically with a skin biopsy1. Immunohistochemical analysis of skin biopsy tissue is a useful investigation in pancytopenic patients presenting with unexplained rashes. The fatality rate has been reported to be greater than 90% 8. Severe systemic infections are the most common cause of death, which often occurs within 3 to 4 weeks from the time of the implicated transfusion. Preterm infants Neonates receiving exchange transfusion Low / Theoretical Risk HIV/ AIDS Corticosteroids, antithymocyte globulin, cyclosporine and growth factors have all been used with minimal success in the treatment of TAGVHD. Because of the lack of effective treatment regimens, TA-GVHD should be prevented by pretransfusion irradiation of all blood products administered to patients at risk. Irradiation inhibits proliferation of donor lymphocytes but has no significant adverse effect on red cell, platelet, or granulocyte function1. Healthy term neonates In the differential diagnosis, viral infections leading to bone marrow suppression, adverse drug reactions and hematologic malignancies were considered, because the features characterizing GVHD may mimic drug reactions or viral infection. Some viral infections were excluded as the ELISA test results were negative. Examination of the bone marrow aspiration and biopsy specimen excluded any hematologic malignancy. Although the patient had used nonsteroidal antiinflammatory drugs for months, repeated preoperative total blood counts (before transfusion) were within normal limits. Therefore, bone marrow suppresion due to drug use was also excluded. Based primarily on case reports and reviews, a number of immunosupressed and immunocompetent patient groups can be stratified according to risk of developing TA-GVHD 8 (Table I). Although our patient was presumably immunocompetent, because of transfusion from blood relatives, he was at high risk of developing GVHD according to this categorization. Of the possible diagnoses, TA-GVHD would most comprehensively explain the clinical picture in this patient. Since the effective standard therapy of TA-GVHD has not been established, the prevention by gamma irradiation of cellular blood components is most important. All blood products are irradiated in the blood banking center in our hospital. But the patient had been transfused with blood obtained from another blood bank. Any unexpected symptoms in a transfusion recipient should at least be considered as a transfusion reaction and be evaluated. The α-subunit has a binding site for GTP and GDP and an intrinsic GTPase activity. The β- and γ- subunits exist as a covalently bound complex and are only active in this form. All three subunits show great diversity: currently at least 20 different genes for α-subunits, 5 for β-subunits and 12 for γ-subunits are known in mammals. Distribution of G-proteins is heterogenous : some G-proteins are ubiquitous, whereas others only occur in specialized tissue. The α-helical domain which can be regarded as a large insertion to the GTPase domain is connected to the GTPase domain by two linker regions. Secondary structure of the unit is completely helical and a long central α-helix (αA) is surrounded by 5 other short α-helices (αB- αF). Specificity of G protein function is mostly determined by the α-subunit: the α-subunit carries out the specific interaction with receptors preceding in the signal chain and with the subsequent effector molecules 3. Recent studies have also shown that G β γ-subunits also interact with their specific effectors such as PLCa isoforms and type I adenylyl cyclase4. All guanine-nucleotide binding proteins have five consensus sequences around the nucleotide binding pocket: the GXGCCGKS motif (where X is any amino acid) in the β, γ phosphate binding loop (P-loop); the N/TKXD motif responsible for the interaction with the nucleotide base; the Mg2+ - binding sequence DXXG ; a second guanine ring binding site , TCAT, that is homologous to the ras p21 sequence TSAK. Structural Basis of the Activation of the αSubunit: The main conformational change that takes place during the transition form the inactive (GDPbound) to the active (GTP-bound) form is a change in the positions of phosphates and Mg2+. Overall Structure: In both active and inactive conformations, the guanine nucleotide ring and the ribose unit stay deeply buried between the GTPase and α-helical domains, while Mg2+ and b-phosphates of GDP move from a surface-exposed position inward to a region protected between the domains. The α-subunit consists of two domains: 1) A domain similar to ras p21, a monomeric GTP-binding protein responsible from the signalling processes leading to cell multiplication and differentiation upon activation by receptor tyrosine kinases. This domain is called the "GTPase" domain since it is common in all GTPases. 2) An additional a-helical domain that is unique to heterotrimeric G-proteins. The GTPase domain consists of six-stranded βsheets surrounded by five a-helices (Fig. 1). This domain contains common regions involved in GTP-binding of all GTPases, the phosphate binding loop (P-loop) , the Mg2+-binding residues and two guanine ring binding sites4. This movement is a result of conformational changes around the γ-phosphate of GTP , localized to three regions called switch I, II and III. Switch 1 spans through Ser 173-Thr183, including Arg 174 and Thr 177. Arg 174 is the possible site for ADP-ribosylation and plays a key role in stabilizing the transition state and hydrolysis of GTP. The region referred to as switch 2 includes Gly 199 and α-helix 2. Switch 3 is a loop segment spanning residues between Asp 227 - Arg 2384. After activation, switch I moves toward the g phosphate, bringing the side chain of Thr 177 (in Gat) close to the γ-phosphate and Mg2+. Mg2+ is required both to locate γ-phosphate properly and to stabilise guanine nucleotide complex 8. The hydrogen bond established between the main chain NH of Gly 199 and γ-phosphate of GTP strecthes and rotates the switch II, which includes both the loop containing Gly 199 and helix α2.The switch III region has no direct interactions with γphosphate, but by interaction with switch II helps to fix the new conformation (Fig. 2)8. It is likely that direct interactions of receptor with the G protein α subunit are necessary to facilitate dissociation of GDP. The receptor possibly interacts with amino terminus of G α, thereby distorting the α-/ β γ- surface11. Interaction of the receptor with the carboxyl terminus of α is clearly important. The extreme Cterminal end of G α, the last five residues in particular, has been implicated as an important mediator of receptor-G protein interaction. Interaction with β γ-subunits: The β-subunit shows a propeller structure composed of seven motifs, each comprising fourstranded antiparallel sheets. Each of the seven motifs is called a β-blade. Contact region of αsubunit is opposite to where G γ subunit binds; so there is no contact between α- and γ-subunits. There are two major contact sites between G α and G β. The first is the myristoylated or palmitoylated amino terminus of G α. The larger and more important site is the interface between the G α switch regions I and II and one of the G β's electronegative faces9. The N-terminal of isolated G α is disordered, but it becomes stabilized when it interacts with β-subunit. The overall structures of both α- and β γ-subunits have been preserved, except for the N-terminal region of the α-subunit and changes around switch regions10. The role of the switch II region is especially important in interaction with β γ. Conserved residues like Gly 199 and Gln 200 stabilize the transition state for GTP hydrolysis. In the β γbound form those switch regions lose their flexibility and firmly bind GDP. The replacement of GDP for GTP causes local but crucial conformational changes around switch I and II, thereby providing the appropriate conditions for release of G α from G β γ8. Key residues in α2 and a4 helices have been found to be involved in G-protein coupling to its receptors. Mutations in residues in tha α4/ α5 loop, especially residues at the C-terminal end of the α4 helix, have suggested a role for these residues in receptor-mediated activation of G α subunits. The structure of bovine rhodopsin, a G protein coupled receptor was determined by Palczewski et al. only four years ago 19. Solution-based techniques such as electron paramagnetic resonance, small angle X-ray scattering analysis, circular dichroism or highresolution nuclear magnetic resonance are additional structural tools that can be employed to analyse protein-protein interactions when crystallisation is difficult for the protein of interest. These tools can be used in combination with biochemical and mutational approaches to provide deeper understanding of those mechanisms. Interaction with Effectors: Each G α-family activates its specific effectors. Cocrystallization studies of G αs and the catalytic domains of adenylyl cyclase (AC) have identified specific contact regions within G αs at the α2helix and the α3- β5 loop14. In addition, the α4- β6 loop of G αs also plays a role in adenylyl cyclase activation15. Using G αi/G αs chimeras, it was demonstrated that the carboxyl terminus of G αs, specifically the region G αs 235-356, contained the adenylyl cyclase activating region and mutation of residues within this region caused a decrease in the ability to stimulate adenylyl cyclase16. Structural determinants for regulation of cGMP phosphodiesterase (PDE) by a G αt/i chimera and RGS9 were reported in an X-ray crystallographic study at 2.0 Å resolution by Slep et al. 17. Future Prospects for G-protein Signaling: The determination of crystal structures of Gproteins in various activation states have revealed the activation mechanism of G-proteins in detail. Comparison of G αt-GDP and G αt-GTP γS crystal structures has indicated to the presence of three flexible regions, which become more rigid and ordered upon GTP activation. The structures of extreme N- and C-terminals are not well known, since they are generally disordered or removed to prevent posttranslational 13 modifications .
