2 passport sized (3.5 cms x 4.5 cms) coloured photographs with white background taken recently. On the photograph, the applicant’s face- chin, forehead, nose, eyes,
eyebrows and the mouth- should be clearly visible. (Size is top of hair to chin must be 3/3,5 cm)

Proof of sponsorship and/ or accommodation. If the person is going to stay at a hotel, the hotel reservation or any other proof of accommodation.

b) Düzenli gelir kanıtı örneğin son 3 aylık maaş bordrosu (ya da benzeri)
Proof of means of subsistence: e.g.
a) Bank account statement on the account turnover during the last three months,
b) Proof of regular income, e.g. salary slips of the last three months (or similar)
Başvuru sahibi çalışıyor ise; işvereninden izin mektubu ve/veya izinli olduğunun onayı (izin yazısında başvuru sahibinin ne görevle ve ne kadar süredir

çalıştığı, Danimarka/Schengen ülkesini ziyaretinden sonra çalışmaya devam edeceği bilgisi eklenmelidir.) , son 3 ayın maaş bordrosu, işe giriş bildirgesi ve hizmet dökümü, işverenin faaliyet
belgesi,oda sicil kaydı,ticaret sicil gazetesi,vergi levhası,imza sirküleri.
If the applicant is employed: permission letter from the employer and/or approval for leave ( tasks performed by the applicant, the period s/he has been working and
a statement that s/he will return to work after visiting Denmark/ Schengen countries should be included), salary slips of the last three months, SGK statement of
employment, and SGK statements,copy of the company registrationin chamber of commerce and copy of the bulletin of the trade register,tax registration certificate
of the company ;copy of article in official media regarding the formation of the company,copy of signature circulars to verify the authority to sign the documents.

If applicant is a farmer: farmer certificate issued by the chamber of agriculture.
Başvuru sahibi işveren ise; Oda Sicil Kaydı, Faaliyet Belgesi, Ticaret Sicil Gazetesi, Vergi Levhası,İmza sirküleri.

If the applicant is self-employed: Company registration in chamber of commerce, e.g Copy of Chamber of Commerce Registration and Certificate of Activity (Oda Sicil Kayıt Örneği ve Faaliyet Belgesi)
and copy of the bulletin of the trade register and statement of taxes payment, e.g Tax Chart (Vergi Levhası),copy of sigbature circulars to verify the authority to sign the documents.
Pasaport Fotokopisi : Son 10 yıl içerisinde alınmış geçerli bir pasaport ve işlem görmüş sayfaların renkli fotokopisi (en az iki sayfası boş olmalı). Varsa eski
pasaportlar ve A4 boyutlu renkli fotokopileri (işlem görmüş tüm sayfalar).
A valid passport issued within the last 10 years and a A4-sized coloured photocopy. If available old passports and coloured copies (copies of all non-blank pages).
