Bu form 01.09.2013 tarihinde yürürlüğe giren “Bankalar ile Bireysel Müşterileri Arasında İmzalanacak Sözleşmelerin
Şekil ve İçeriğinde Yer Alması Gereken Asgari Hususlar ile Sözleşmelerin Uygulanacağı İşlemlere İlişkin Usul ve
Esaslar Hakkında Tebliğ” kapsamında Bankamız tarafından sunulan temel bankacılık ürün ve hizmetlerde uygulanan
ücret, masraf, faiz ve komisyonlar hakkında müşterilerimizin açık ve doğru bilgilendirilmesi amacıyla hazırlanmıştır. /
This form has been prepared in order to notify in clear and correct manner our customers about the basic banking product
as well as the price, expense, interest and commission applied to the services of our Bank in the scope of “the notification
regarding the Minimum Matters to be included in the form and content of agreements to be concluded between the Banks
and retail customer as well as procedures and principles concerning the transactions to be applied to these agreements”
entered into force on 01.09.2013.

opened and cancelled on the same day./ (2) At fee assessment, it shall be necessary to wait for the investor to receive
his registration number and relevant investor password. For foreign sending of registration and password as printed
document, the post fee shall be collected based on the amount calculated by adding half of the domestic post fee to
the PTT post cost./ (3) CRA (Central Registry Agency) account maintenance fee shall be valid for only market
valuable custody balance equal or above 1000 TL . (4) / No additional fee other than TEFAS rates will be charged
from investment fund participation share purchase sale transactions realized at TEFAS platform /(5) The minimum
amount to be requested per transaction is 1 piaster (6) (7) (8) (9) .The rates are annual.

EFFECTIVE TERM OF THE FEES AND NOTIFICATION OF CHANGES:
Bir takvim yılı içinde bu formda yer alan ücretlerde, Türkiye İstatistik Kurumunca bir önceki yılsonu itibariyle
açıklanan yıllık tüketici fiyatları endeksi artış oranının 1,2 katını geçmeyecek ücret değişiklikleri 30 gün önceden
tarafınıza e-posta, ATM, telefon, kısa mesaj veya benzeri araçlarla bildirilecektir. Bildirim tarihinden sonraki 15 gün
içinde ürünün veya hizmetin kullanımından vazgeçme hakkınız bulunmaktadır. Bu hakkın kullanılması halinde
tarafınızdan hiçbir şekilde yeni döneme ait ek bir ücret ve/veya komisyon tahsil edilmeyecektir. Ürün veya hizmetin
kullanımıyla ilgili ücret ve faiz değişikliğinin kabul edilmemesi durumunda Bankamızın, tarafınıza bu hizmeti vermeyi
durdurma hakkı saklıdır.

Bu formda yer alan ücretlerde yıllık tüketici fiyatları endeksi artış oranının 1,2 katı ve üzerindeki artış
değişiklikleri için ayrıca talebiniz alınacaktır. Süreklilik arz etmeyen anlık işlem ve hizmetlerden faydalanmak
istemeniz halinde, işlem anında henüz ücret tahsil edilmeden önce, tarafınıza ücret bilgilendirilmesi yapılacak ve
onayınız alınacaktır.
“Yatırım Ürünleri Merkezi Kayıt Kuruluşu Ücretleri” ve “Yatırım Ürünleri ve Türev Ürünler Belge Gönderim
Ücretleri” yukarıda belirtilen bildirim ve onay alma yükümlülüklerinden muaftır. /

Changes in the fees indicated in this form up to 1.2 times of the annual consumer price index announced by the Turkish
Statistical Institute as of the year-end of the previous year in a calendar year will be notified to you 30 days in advance,
through e-mail, ATMs, phone, short message and similar means. You have the right to abandon your usage of the
product or service within 15 days following the date of announcement of such fee. If you exercise this right, no
additional fee and/or commission will be collected from you pertaining to the new period. If you do not accept the
relevant fee and interest changes concerning the usage of the product or service, our Bank reserves the right to suspend
the provision of such service to you.

Your further request will be asked in case of an increase of fees indicated in this form equal to or more than 1.2 times
of annual consumer price index increase rate If you request to benefit from the discontinuous instant transactions and
services, you will be informed about the fee and your approval will be asked before collecting the fee at the time of the
transaction.
“Investment Products Central Registry Agency Fees” and “Investment Products and By-Products Document Post Fees”
are exempted from the informing and receiving approval stated here above.
TAHSİLAT ŞEKLİ / PAYMENT CONDITIONS:
Masraf ve komisyon tahsilatları ürün ve hizmet tipine göre, nakden veya hesaben veya müşterinin talebine ve onayına
bağlı olarak kredi kartı hesabına borç kaydedilmek veya Ek Hesap* limitinden tahsil edilmek suretiyle yapılır. Yukarıda
özetlenen konular dışında her türlü detaylı bilgi Bankacılık İşlemleri Sözleşmesi’nde yer almaktadır. Dikkatle
inceleyebilmeniz ve anlaşılmayan konularda Bankamızdan detaylı açıklama talep edebilmeniz amacıyla bu formun bir
nüshası Bankacılık İşlemleri Sözleşmesine ek olarak tarafınıza teslim edilmiştir. Okuyup uygun bulmanız durumunda,
bu 2(iki) nüsha düzenlenmiş formu imzalayarak 1(bir) nüshasını saklayınız.
*Ek Hesap, vadesiz mevduat hesabınıza tanımlanacak bir kredi limitidir.
Mesafeli olarak uzaktan iletişim aracı ile işlem yapılması durumunda bu doküman ıslak imzaya gerek olmadan
elektronik ortamda onaylanır. Uzaktan iletişim aracı ile yapılan mesafeli işlemler dışında yapacağınız işlemlerde yazılı
olarak ıslak imza atmak suretiyle işleme taraf olmanız mümkündür. /

According to the type of product and service and upon the demand and approval of the customer, the fees and
commissions can be collected in cash or paid on account or by debiting the debt on the credit card account or from the
Overdraft* limit. Any and all kinds of detailed information apart from the above mentioned matters are indicated in the
Banking Transaction Agreement. This form has been submitted for you as annex of Banking Transaction Agreement
in order to allow you demand detailed explanation from our Bank for incoherent matters. After having read and
understood this form, please sign this form issued in 2(two) copies and keep 1(one) copy.
*Overdraft is a credit limit to be assigned to your demand deposit account.
In case of transaction done through remote communication mean, this form shall be approved in electronic environment
without wet signature. For transactions to be done apart from the transactions fulfilled through remote communication
mean, you can become a party of the agreement only after putting your wet signature.
