Vize Başvuru Form u: Eksiksiz, imzalı, gerçeğe uygun doldurulmuş başvuru formu.
Visa Application Form: Completed and signed visa application form.

İki adet fotoğraf : Standartlara uygun (yüz görünür), yeni çekilmiş(6 aydan daha eski olmayan), arka fon beyaz iki adet biyometrik fotoğraf (35X4 5 ebat)
Two recent photographs: Not older than 6 months, 34x45 in colour, preferably biometric and with white background

Pasaport Fotokopisi : Son 10 yıl içerisinde alınmış geçerli bir pasaport ve işlem görmüş sayfaların renkli fotokopisi (en az iki sayfası boş olmalı ). Varsa eski
pasaportlar ve A4 boyutlu renkli fotokopileri (işlem görmüş tüm sayfalar).
A valid passport issued within the last 10 years and a A4 -sized coloured photocopy. If available old passports and coloured copies (copies of all non-blank pages).

Sarı sağlık sigorta kartı veya ehliyeti (fotokopi).
Yellow Card (sygesikringsbevis) or driving license, where applicable.

Uçak Bileti rezervasyonu
Flight ticket reservation.
