HOW TO USE THIS FORM
Please submit this form to initiate an employee’s leave of absence, update details, or return an employee from a leave. Only applied to
leave types that are status changing
This form is used for submitting those absence types which require HR/Manager approvals or impact payroll. This form is not used for
processing absence requests that can be done using Employee-Self-Service (ESS). Please complete this form with all required
