Abstract
Beginning with Freud’s departure from his initial seduction theory, psychoanalytic
theory has been split over how to think about childhood sexual abuse. Whereas
one side of the debate emphasizes the actuality and reality of sexual abuse, the
other side of the debate doesn’t concern itself with what happened in reality and
instead focuses on the phantasied symbolizations and elaborations of certain
sexual abuse dynamics. While actual trauma does impair the capacity to
fantasize, there are many other patients who haven’t personally experienced a
traumatic episode, yet who display similar dynamics or bring about similar
transference and countertransference dynamics in psychotherapy to the patients
who have actually gone through trauma. Psychotherapists can only make sense
of the rapidly oscillating transference and countertransference paradigms – the
main hallmark of the patients with sexual abuse dynamics – insofar as he/she
stays with the phantasy in the room. Since what really happened to the patient is
impossible to establish, it is through the phantasy in the room that the
psychotherapist really gets to know the patient. In this context, certain
treatment paradigms are discussed in phantasy’s predominance over reality in
psychotherapy, and subsequently a psychoanalytically oriented psychotherapist’s
functions are highlighted; namely the psychotherapist who guides the patient in
learning to dream and think creatively about one’s emotional difficulties.

Freud baştan çıkarma varsayımından uzaklaştığından beri psikanalitik teori çocuk
cinsel istismarı konusunda ikiye bölünmüştür. Tartışmanın bir tarafı çocuk cinsel
istismarının hakikatı ve gerçekliği üzerine odaklanırken, diğer taraf istismarın
gerçekliği ve hakikati yerine cinsel istismar dinamiklerinin düşlemlenmiş
sembolleştirmelerine ve elaborasyonlarına odaklanır. Bir yanda gerçekten
yaşanmış travma düşlemleme kapasitesini zayıflatırken, gerçek bir travma
yaşamamış bir çok hasta psikoterapide travma yaşamış olanlara benzer aktarım
ve karşıaktarım dinamikleri sergilerler. Psikoterapistler cinsel istismar
dinamiklerine sahip hastaların sık dalgalanan aktarım ve karşıaktarım
paradigmalarını ancak psikoterapi odasındaki düşlemde kalarak anlayabilirler.
Hastaya gerçekte ne olduğunu bilmek hiç bir zaman mümkün olmadığından
psikoterapistler psikoterapi odasındaki düşlemin yol göstericiliğiyle hastayı
gerçekten tanıyabilirler. Bu bağlamda bu projede psikanalitik psikoterapide
düşlemin gerçekliğe ağır basmasından ve psikanalitik yönelimli psikoterapistin
işlevlerinden bahsedilmiştir ki bu psikoterapist hastanın rüyalaştırma yetisine ve
problemleri hakkında yaratıcı bir şekilde düşünmesine yardımcı olandır.
