2 passport sized (3.5 cms x 4.5 cms) coloured photographs with white background taken recently. On the photograph, the applicant’s face- chin, forehead, nose, eyes,
eyebrows and the mouth- should be clearly visible. (Size is top of hair to chin must be 3/3,5 cm)

Signed invitation letter from the reference in Denmark (including proof of accommodation)
Davet eden kişinin pasaport fotokopisi (davet eden kişi Danimarka vatandaşıysa) ya da davet eden kişinin oturum izni fotokopisi (davet eden kişi Danimarka
vatandaşı değilse). Başvuru sahibi nişanlısını, kız arkadaşını veya erkek arkdaşını ziyaret ediyorsa fotoğraf eklenmelidir.
Passport photocopy of the inviting person (if the inviter is a Danish citizen) or a copy of host’s Danish residence card (if the inviter is not a Danish citizen). If the
applicant is visiting his/her fiancé or boy/ girlfriend a photo taken together must be added.

Proof of means of subsistence: e.g.

Başvuru sahibi çalışıyor ise; işvereninden izin mektubu ve/veya izinli olduğunun onayı (izin yazısında başvuru sahibinin ne görevle ve ne kadar süredir

çalıştığı, Danimarka/Schengen ülkesini ziyaretinden sonra çalışmaya devam edeceği bilgisi eklenmelidir.) , son 3 ayın maaş bordrosu, işe giriş bildirgesi ve hizmet dökümü, işverenin faaliyet
belgesi,oda sicil kaydı,ticaret sicil gazetesi,vergi levhası,imza sirküleri.
If the applicant is employed: permission letter from the employer and/or approval for leave ( tasks performed by the applicant, the period s/he has been working and
a statement that s/he will return to work after visiting Denmark/ Schengen countries should be included), salary slips of the last three months, SGK statement of
employment, and SGK statements,copy of the company registration in chamber of commerce and copy of the bulletin of the trade register,tax registration certificate
of the company ,copy of article in official mediaregarding the formation of the company,copy of signature circulars to verify the authority to sign the documents.

Başvuru sahibi çiftçi ise, ziraat odasından çiftçi sertifikası
If applicant is a farmer: farmer certificate issued by the chamber of agriculture.
Başvuru sahibi işveren ise; Oda Sicil Kaydı, Faaliyet Belgesi, Ticaret Sicil Gazetesi, Vergi Levhası ,İmza sirküleri.

If the applicant is self-employed: Company registration in chamber of commerce, e.g Copy of Chamber of Commerce Registration and Certificate of Activity (Oda Sicil Kayıt Örneği ve Faaliyet Belgesi)
and copy of the bulletin of the trade register and statement of taxes payment, e.g Tax Chart (Vergi Levhası).,copy of signature circulars to verify the authority to sign the documents.
Başvuru sahibi emekli ise, emekliliğin kanıtını gösterir belgeler (emekli maaşını aldığı banka hesap hareketleri veya )
If applicant is retired: proof of pension (either bank account statement or emeklilik cüzdanı).
Başvuru sahibi öğrenci ise, İngilizce ve original öğrenci belgesi
If the applicant is a student: original student certificate in English.
Başvuru sahibi 18 yaşından ufaksa ve tek başına seyahat ediyorsa: Noterden onaylı muvaffakatname. Sadece bir ebeveyn çocuğa eşlik ediyorsa, diğer

ebeveynin noterden onaylı muvaffakatnamesi gereklidir. (Bu durum eğer ebeveyn tek ebeveynse veya çocuğun velayeti varsa geçerli değildir ) Eğer her iki ebeveyn de yolculuk ediyorsa
muvaffakatnameye gerek yoktur.
If the applicant is a minor/ under the age of 18 and travelling alone: both parents’ Letter of Consent approved by the Public Notary. If one parent is
accompanying the child: the other parent’s consent is needed. (This condition shall not apply if the travelling parent is a single parent or holds the
parental authority alone). If both parents travel together with the child: there is no need for the consent.

Pasaport Fotokopisi : Son 10 yıl içerisinde alınmış geçerli bir pasaport ve işlem görmüş sayfaların renkli fotokopisi (en az iki sayfası boş olmalı). Varsa eski
pasaportlar ve A4 boyutlu renkli fotokopileri (işlem görmüş tüm sayfalar).
